Prevention of postoperative adhesion formation by a protein kinase inhibitor SU5614 in a rat uterine horn model

Abstract
Objective: Our purpose was to examine the effect of SU5614 (5-Chloro-3-[(3,5-dimethylpyrrol-2-yl)methylene]-2-indolinone), which inhibits VEGF-dependent phosphorylation of the VEGFR-2 and its biological effects, on the development of postoperative adhesion formation in a rat model. Institution: Department of Obstetrics and Gynecology, Mersin University Materials and Methods: Thirty female rats were randomly assigned (by random number table) into three groups and a standardized peritoneal injury on the anti mesenteric surface of each right uterine horn was made for each. The control group (Group A) was not given any medication. 0.2 ml of l _M SU5614 and 0.2 ml of 10 _M SU5614 were instilled onto abdominal cavity of the rats in the Group B and Group C respectively. Adhesion scores were determined by a laparotomy on postoperative day 21. We also scored the immunohistochemical staining of adhesion biopsy specimens with VEGF antibody. Results: All rats in control group had adhesions. Intra-abdominal adhesions were present in 7 out of 10 rats in both Group B and Group C. The rats in Group B and Group C had significantly lower adhesion scores than the rats in Group A (control group) (p<0.05). There was no significant difference between the Group B and Group C. The biopsies taken from adhesions in Group A had significantly higher immunohistochemical staining scores than in Group B (p<0.05). There was no significant difference between Group A and C as for between Group B and C in staining scores. Conclusion: Our results supports that blockage of the biologic effects of VEGF in the inflammatory milie'u, which occurs postoperatively at the operation site, might limit the adhesion formation after pelvic surgery. However, further studies are needed to exactly clarify the effect of VEGF and its opposition on the pathophysiology of adhesion formation.
Özet
Amaç: Biz bu çalışmamızda, bir VEGF reseptör blokörü olan SU5614’un (5-Choloro-3-[(3,5-dimethylpyrrol-2-yi)methylene]-2-indolinone) postoperatif adezyon gelişimi üzerindeki etkisini deneysel rat modeli üzerinde araştırdık. Çalışmanın Yapıldığı Yer: Mersin Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum Anabilim Dalı. Materyal ve Metod: Otuz adet, dişi Whistar albino rat, randomize olarak, 10’ar rattan oluşan 3 gruba ayrıldı ve her bir ratta sağ uterin hornun antimezenterik yüzeyi üzerinde standart peritoneal hasar oluşturuldu. Kontrol grubunda (Grup A) hiçbir şey uygulanmazken, Grup B ve Grup C’de sırasıyla intraperitoneal olarak 0.2 ml 1 _M ve 0.2 ml 10 _M SU5614 bırakıldı. Postoperatif 21. gün’de laparatomi ile adezyon oluşumları değerlendirildi. Adezyon biyopsileri de immunohistokimyasal boyama ile VEGF açısından skorlandı. Bulgular: Kontrol grubundaki tüm ratlarda adezyon gözlendi. Grup B ve Grup C de ise her 10 rattan 7 sinde adezyon izlendi. Grup B ve C’deki ratlarda, Grup A’daki ratlara göre anlamlı olarak daha az adezyon saptandı (p<0.05). Grup B ve Grup C’deki adezyon skorları arasında istatistiksel fark izlenmedi. Grup A’da bulunan ratlardan alınan adezyon biyopsilerinin immunohistokimyasal boyamalarında Grup B’ye göre anlamlı olarak daha fazla reaksiyon saptandı (p<0.05). Boyama skorları açıdan diğer gruplar arasında fark görülmedi. Sonuçlar:Bu çalışmanın sonuçları, postoperatif dönemde operasyon bölgesinde oluşan inflamatuar ortamda VEGF nin biyolojik etkisinin engellenmesinin, pelvik cerrahi sonrası adezyon oluşumunu sınırlayabileceğini göstermektedir. Bununla birlike VEGF nin kesin etkisinin ortaya konması ve adezyon formasyonu patofizyolojisindeki yerinin anlaşılabilmesi için daha ileri çalışmalara ihtiyaç vardır
