Relationship of constipation and autonomic neuropathy in patients with non-insulin dependent diabetes mellitus and the effects of cisapride on colonic transit time

Abstract
In this study, the relationship of constipation (C) and autonomic neuropathy (AN), and the effects of cisapride, a prokinetic drug, on colonic transit time (CTT) have been investigated in 27patients with non-insulin dependent diabetes mellitus (NIDDM). Fourteen patients were male, and 13 patients were female. Mean age was 63.6 ± 8.1 years (range 51-81 years). AN symptoms were observed in all patients except one case. AN scoring was made for all of the patients. Radioopaque substances were used for measuring of CTT. Cisapride was given 10 mg qid for 20 days in patients with prolonged CTT. CTT was remeasured on 21 st day. Patients were subdivided into groups according to the presence of C and AN. G-I (n=8): C (+), AN (+) G-A: G-I + G-II G-II (n=9): C (+), AN (-) G-B: G-III + G-IV G-III (n=4): C (-), AN (+) G-C: G-I + G-III G-IV (n=6): C (-), AN (-) G-D: G-II + G-IV. CTT and glycemia values of patients both in Group-I and -II were significantly lower (p<0.05) after treatment. CTT in Group-A after teratment was still longer than Group-B (p<0.05). Shortening of CTT in Group-II after treatment was more prominent than Group-I (p>0.05). CTT in Group-C at the beginning of the treatment was longer than Group-D. A positive correlation was found between the diabetic age and CTT of the patients. The same correlation was observed in glycemic control and CTT. The relationship between AN and CTT, on the other hand, was only supportive. In conclusion, the presence of chronic constipation and AN was only coincidental and poor metabolic control was the etiologic factor for both condition. Cisapride significantly decreases CTT in diabetic patients with constipation (p<0.05) free from AN, but not as much as in diabetic patients without constipation. Also presence of AN in diabetic patients is shown to prolong CTT(p<0.05).

Bu çalışmada diyabetik hastalarda konstipasyonun (KP) otonom nöropati (ON) ile ilişkisi ve prokinetik bir ajan olan Cisapride'in kolon transit zamanı (KTZ) üzerindeki etkisi araştırıldı. 14'ü erkek toplam 27 hastanın yaşları 51-81 yıl (63.6 ±8.1 yıl) arasında değişiyordu. Biri dışında tüm hastalarda ON semptomları mevcuttu. Tüm hastalarda ON skorlaması uygulandı. Hastalarda KTZ radyoopak marker kullanımı ile belirlendi. KTZ 70 saatten uzun olanlarda 20 gün süreyle 4x10 mg Dispeprid tablet kullanımından sonra ilacı kesmeksizin KTZ ölçümü tekrarlandı. Hastalar KP ve ON mevcudiyetine göre alt gruplara (G) ayrıldı: G-I (n=8): KP (+), ON (+) G-A (n=17): G-I + G-II G-II (n=9): KP (+), ON (-) G-B (n=10): G-III + G-IV G-III (n=4): KP (-), ON (+) G-C (n=12): G-I + G-III G-IV (n=6): KP (-), ON (-) G-D (n=15): G-II + G-IV I ve II. gruplarda tedavi sonrası KTZ ve glisemi değerleri önemli oranda azaldı (p 0.05). Çalışma başlangıcında G-C'de saptanan KTZ, G-D'ye oranla daha uzundu (p>0.05). Hastaların diyabet süresi ile KTZ arasında pozitif bir ilişki vardı. Aynı ilişki glisemi regülasyonu ile KTZ arasında da saptandı. ON ile KTZ arasında ise birbirlerine katkıda bulunan bir ilişki gözlendi. Sonuç olarak, diyabetik hastalarda kronik konstipasyon ile otonom nöropatinin birlikteliği raslantısal olup, her ikisi de kötü metabolik kontrol ile şiddetlenmektedir. Cisapride'in konstipe diyabetik hastalarda KTZ'yi ON'den bağımsız olarak anlamli oranda kısalttığı (p<0.05), ancak konstipasyonsuz diyabetiklerdeki KTZ seviyesine eriştiremediği söylenebilir. Ayrıca, ON mevcudiyetinin diyabetik hastalarda KTZ uzamasına (p<0.05) neden olabileceği kanaatine varılmıştır.
