The effects of non-steroidal anti-inflammatory drug ( NSAID ) use on serum gastrin, pepsinogen-1 levels and Helicobacter pylori status

Abstract
Serious adverse events can be seen in the gastrointestinal tract and kidneys of patients treated with NSAIDs. The purpose of this study was to demostrate whether a relationship exists between NSAID induced gastrointestinal mucosal injury and the presence of H. pylori and correlation of serum gastrin and pepsinogen-1 levels with the development of mucosal injury. The study included 30 patients (11 female and 9 male) with a mean age of 36,6 (18-65) years. Who were randomised into three comparable groups and either aspirin 300mg/day, nabumetone, 1000 mg/day, or placebo was given orally for 10 days. Upper gastrointestinal tract endoscopy was performed and symptoms evaluated in each group before and after the treatment period. H. pylori was found to be positive in 24 (80%), patients prior to treatment. Gastrointestinal symptoms before and after 10 days of NSAID treatment were not significantly different (p>0.05) but patients treated with either aspirin (COX-1 inhibitor) or nabumetone (COX-2 inhibitor) had a significant (p<0.05) increase in mucosal injury as demonstrated by endoscopy, when compared to pretreatment findings. No difference was found between the severity or type of mucosal lesions induced either by aspirin or nabumetone. Fasting serum gastrin and pepsinogen-1 levels before and after NSAID treatment were found to be similar. NSAID induced mucosal changes were seen more frequently (p<0.05 ) in patients with H. pylori positi-vity when compared to those with negative values. In summary, short term NSAID treatment induces asymptomatic mucosal changes. The presence of H. pylori has a role in the development of NSAID induced mucosal changes but pathophysiologic mechanism is not through changes in serum gastrin or pepsinogen-1 levels.

Nonsteroidal antiinflamatuvar ilaç (NSAİİ) kullanımına bağlı olarak özofagus, mide, duodenum ve böbreklerde önemli yan etkiler görülebilir. Sunulan bu çalışmanın amacı, Helicobacter pylori (H. pylori) infeksiyonunun NSAİİ kullanımına bağlı oluşabilecek gastroduodenal mukozal hasar oluşumundaki etkisinin belirlenmesi ve gelişen bu hasarın açlık serum gastrin ve pepsinojen-I seviyeleriyle ilişkisinin ortaya konmasıdır. Çalışmaya yaş ortalamaları 36.6 (18-66 yaş) olan 11E ve 19K toplam 30 olgu dahil edildi. Bu kişiler üç eşit gruba bölünerek her gruba 10 gün süreyle Aspirin 300 mg/gün, plasebo ve Nabumeton 1000 mg/gün ağız yolu ile verildi. Olgularda NSAİİ veya plasebo öncesi ve sonrası olmak üzere dispeptik ve endoskopik skorlamalar yapıldı. Ayrıca, H. pylori varlığı ile açlık serum gastrin ve pepsinojen-I seviyeleri araştırıldı. Başlangıçta 30 hastanın 24'ünde (%80) H. pylori(+) iken; 6'sında (%20) H. pylori(-) bulundu. NSAİİ verilmesi ile gastrointestinal semptomlarda anlamlı artış saptanmadı (p>0.05). Buna karşılık endoskopik olarak hem COX-1 inhibitörü olan Aspirin verilen olgularda, hem de selektif COX-2 inhibitörü olarak bildirilen Nabumeton verilen olgularda anlamlı derecede hasar tespit edildi (p 0.05). H. pylori pozitifliği ile NSAİİ kullanımı sonrası mukozal hasar gelişimi arasında anlamlı ilişki saptandı (p<0.01). Sonuç olarak kısa süreli NSAİİ kullanımının asemptomatik gastroduodenal mukozal hasar oluşturduğu, H. pylori infeksiyonunun gelişen hasar üzerine etkili olduğu, ancak bu hasarın belirlenmesinde açlık serum gastrin ve pepsinojen-I seviyelerinin iyi bir belirleyici olmadığı saptandı.
