Gastric mucous cell metaplasia of the duodenum and the histopathology of the antrum

Abstract
Helicobacter pylori associated gastritis is very strongly associated with duodenal ulceration. Gas¬tric mucous cell metaplasia found in duodenum is a nonspecific mucosal response mainly to acid injury. The aim of this study is to investigate the association of chronic gastritis and duodenal ulceration and also the prevalance and importance gastric mucous cell meta¬plasia in the duodenum. Biopsies from anterior and posterior wall of duodenum and gastric antrum were taken from 20 patients with active duodenal ulcer. We investigated antral gastritis and Helicobacter pylori, duodenit and gastric mucous cell metaplasia in the duo¬denum and also H. pylori in this metaplastic areas. In 95% of cases which showed H. pylori in the gastric an¬trum was acompanied with activity in 70% of cases. Gastric metaplasia was found in 45% of duodenal biop¬sies.
Özet
Helicobacter pilori'ye bağlı kronik gastrit duode¬nal ülserle yakından ilişkilidir. Duodenumdaki gastrik müköz hücre metaplazisi ise temelde asit zedelemesiyle oluşan nonspesifik mukozal yanıttır. Çalışmamızda kronik gastrit-duodenal ülser ilişkisini ortaya koymak ve duodenumdaki gastrik metaplazinin varlığı ile öne¬mini araştırmayı amaçladık. Hastanemiz Gastroentero¬loji kliniğine başvuran ve aktif duodenal ülseri olan 20 hastadan bulbusun anterior ve posterior duvarı ile mide antrumundan biyopsiler alındı. Antrumda gastrit ve H. pylorinin varlığı, duodenumda da duodenit ve gastrik metaplazi ile birlikte bu odaklarda H. pylori'nin varlığı araştırıldı. Olgularımızın %95'inde antrumda saptadığımız H. pylori'ye %70 olguda aktivite eşlik et¬mekte idi. Duodenumda ise olguların %45'inde gastrik metaplazi mevcuttu.
