Effect of morphine, ketorolac and tenoxicam on postoperative small intestinal ileus in rats

Abstract
Despite major advances in the understanding and treatment of disease in many areas of medicine, relatively few advances have been made in the understanding of postoperative ileus. This study was designed to determine the effects of morphine sulfate, ketorolac tromethamine and tenoxicam on small intestinal propulsion and small intestinal myoelectric activity in a rat model of postoperative ileus. İntestinal transit was determined by administering a bolus of radioactive Na251CrO4 via a catheter in the duodenum and following its progression through the small intestine. Intestinal motility was determined by monitoring intestinal myoelectric activity by electrodes implanted on the serosal surface of the proximal small bowel. When ketorolac and tenoxicam were administered preoperatively, they prevented the delay in intestinal transit and inhibition of myoelectric activity seen in postoperative ileus. This study suggests that ketorolac and tenoxicam administered preoperatively may be of value in the prevention and treatment of postoperative ileus.
Özet
Bu çalışma postoperatif ileus oluşturulan rat modelinde morfin sülfat, ketorolak ve tenoksikamın ince barsak propulsif ve myoelektrik aktivitesine etkisini araştırmak üzere planlanmıştır. Tıbbın birçok alanında hastalıkların tanı ve tedavileri konusunda önemli ilerlemeler sağlanmasına rağmen postoperatif ileus konusunda çok az ilerleme sağlanmıştır. Postoperatif dönemde kullanılan birçok analjezik ve antienflamatuar ilacın motiliteye etkisi bilinmemektedir. İntestinal transit duodenuma yerleştirilmiş bir kateter yolu ile verilen radyoaktif Na251CrO4 'ün ince barsakta ilerlemesinin incelenmesi ile saptanmıştır. Barsak motilitesi ise proksimal ince barsak serozal yüzeyine implante edilen elektrotlar ile intestinal myoelekriksel aktivitenin moniterizasyonu ile incelenmiştir. Preoperatif dönemde ketorolak ve tenoksikam verilmesi. postoperatif ileusda görülen intestinal transit ve myoelektrik aktivitede ki inhibisyonu önlemiştir. Morfinin ise etkisi saptanmamıştır. Bu çalışma preoperatif dönemde verilen ketorolak ve tenoksikamın postoperatif ileusun engellenmesi ve tedavisinde değeri olabileceğini göstermektedir.
