Abstract

Acute jejunogastric intussusception (JGI) is a rare postgastrectomy syndrome. The present study describes two cases (Type II and IV) of acute JGI Both were treated surgically following admission to hospital with acute abdominal pain hematlemesis and gastric outlet obstruction. One of them was diagnosed preoperatively by endoscopy and a new radiological finding on plain abdominal x-ray was deseribed (protusian to the mass into the gastric fundal air). In the first case, the invaginadet segment was reduced simply and fixed to the stomach whilst in the second case, intestinal resection was required due to necrosis. It is considered that simple reduction and fixation is a safe and simple procedure in the treatment of acute JGI if there is no necrosis present.

Özet

Akut jejunogastrik invajinasyon nadir bir postgastrektomi sendromudur. Biz burada hematemez ve üst gastrointestinal sistem obstrüksiyonu kliniği ile hastaneye yatırılan iki akut jejuno gastrik invajinasyonlu olguyu (Tip II ve IV) sunuyoruz. Her iki olgu da cerrahi olarak tedavi edilmişlerdir. İlk olguda sıkışan jejenum segmenti basitçe redükte edilip daha sonra fikse edilmiştir.İkinci olguda gelişmiş olan nekroz nedeniyle barsak rezeksiyonu gerekli oldu. Hernekadar bazı cerrahlar jejunogastrik invajinasyonun tedavisinde enteroenterostomiyi önerseler de, biz nekroz gelişmemiş vakalarda redüksiyon ve fiksasyonun basit, güvenli ve yeterli bir tedavi olduğuna inanıyoruz.
