Abstract

Background/aims: Although the upper gastrointestinal endoscopy is a common and safe procedure, it may occasionally cause complications in elderly patients with heart disease. The present study investigated the effects of endoscopy on the cardiovascular system of asymptomatic patients over the age of 50 years with no known heart disease. Methods: The study group comprised 40 patients (28 males, 12 females; mean age: 65±15 years, ranges: 50-78) free of signs and symptoms of any heart disease according to their history, physical examination and ECG. Holter recordings were started one hour before the endoscopic procedure and continued until the end of the first hour. Holter recordings repeated at the same time the next day served as controls. Blood pressures and O2 saturations were obtained before, during, and after the procedure The highest level of blood pressure, and the mean and maximum heart rate were recorded just before and during the examination. Results: The frequencies of premature supraventricular and ventricular beats were higher during the first recordings when compared to the control day. Nevertheless, only the differences in maximum heart rate and frequency of supraventricular beat per hour reached statistical significance (p<0.05). Although silent ischemia was detected in two cases, their coronary angiograms were normal. Conclusion: Our result suggests that upper gastrointestinal endoscopy does not cause cardiac complications and may be safely performed on individuals over the age of 50 years with no known heart disease.

Özet

Amaç: Üst gastrointestinal endoskopisi günümüzde güvenle yapılmasına rağmen nadir de olsa yaşlı ve kalp hastalığı olanlarda çeşitli komplikasyonlara yol açabilmektedir. Bu çalışmada 50 yaş üzerinde olup bilinen bir kalp hastalığı olmayan asemptomatik olgularda endoskopinin kardi-yovasküler sistem üzerine olan etkileri araştırılmıştır. Yöntem: Bu çalışma bilinen kalp hastalığı olmayan, fizik muayenesinde ve istirahat EKG sinde kalp hastalığına ait bulgu olmayan 28 erkek, 12 kadın (yaş ortalaması 65±15, yaş aralığı: 50-78) toplam 40 olgu üzerinde yapıldı. Endoskopik işlemden bir saat önce başlanan Holter kayıtlarına işlemden bir saat sonrasına kadar devam edildi. Ertesi gün aynı saatlerde iki saatlik holter kayıtları alınarak kontrol grubu oluşturuldu. Ayrıca işlem öncesi, işlem boyunca ve işlem sonrası kan basıncı ve oksijen saturasyon takipleri yapıldı. Bulgular: En yüksek kan basınçları, maksimal ve ortalama kalp hızları işlemden hemen önce ve işlem sırasında saptandı. Holter analizinde saatlik supraventriküler ve ventriküler erken vuru sıklığı kontrol grubuna nazaran işlem grubunda daha yüksekti. Ancak sadece maksimal kalp hızı ve saatlik supraventriküler kalp hızı arasındaki fark anlamlıydı (p<0.05). İşlem grubunda iki olguda sessiz iskemi atakları saptanmasına rağmen yapılan koroner anjiografileri normal bulundu. Sonuç: Bu çalışmanın sonunda bahsedilen hasta grubunda üst gastrointestinal endoskopisinin herhangi bir kardiyak komplikasyona yol açmadığı ve güvenle yapılabileceği kanısına varıldı.
