Abstract
Background/aims: Gastric mucosal hexosamine is best indicator of mucin production, which is the first line of gastric mucosal defense There are scarce data on the first line gastric mucosal defenses in patients with cirrhosis. Helicobacter pylori status and changes in gastric mucosal defenses were investigated by measuring gastric mucosal hexosamine content in 30 liver cirrhosis patients without portal gastropathy. Methods: Gastric mucosal hexosamine content was measured by a special spectrophotometric method. Helicobacter pylori status was studied with urease testing and histopathological evaluation of antral endoscopic biopsy specimens. Results: There were 16 male and 14 female cirrhotic patients (mean age 47.76±13.85 years, range: 22-70) and 15 control group subjects (eight male, seven female, mean age 45.93±17.96 years, range: 20-70). According to Child's classification, eight patients were class A, 12 class B and 10 class C. Helicobacter pylori was present in 12 cirrhotics and five control patients. There were six patients in the cirrhosis group who had previously undergone multiple sclerotherapy sessions. Patients within Child C group had significantly lower mucosal hexosamine measurements than that of controls (p<0,05). Similarly, the cirrhotic patients positive for Helicobacter pylori had significantly lower mucosal hexosamine levels than those without Helicobacter pylori infection (p<0,05). Patients with and without a history of previous of endoscopic injection sclerotherapy had comparable mucosal hexosamine levels (p>0,05). Conclusion: This study shows that in progressive stages of cirrhosis, gastric mucosa becomes susceptible to damage, which is indicated by gastric mucosal hexosamine levels. Moreover, Helicobacter pylori infection contributes to gastric mucosal vulnerability by further depressing hexosamine levels.

Amaç: Musin, gastrik mukoza savunma sisteminin ilk basamağını oluşturur ve gastrik mukozanın hekzosamin düzeyi musin üretiminin iyi bir göstergesidir. Siroz hastalarında gastrik mukozanın savunma sistemi hakkında çok az bilgi yayınlanmıştır. Yöntem: Portal gastropati bulunmayan otuz siroz olgusunda Helicobacter pylori varlığı ve, gastrik mukoza savunma sistemindeki değişikliklerin tespiti için gastrik mukozal hekzosamin düzeyi araştırılmıştır. Gastrik mukoza hekzosamin düzeyi özel bir spektrofotometrik yöntemle araştırılmıştır. Helicobacter pylori varlığı ise üreaz testi ve antral endoskopik biyopsi örneklerinin histopatolojik değerlendirmesi ile gerçekleşitirildi. Siroz hasta grubunda 16 erkek ve 14 kadın mevcut olup, yaş ortalaması 47.76±13.85 yıl (22-70) idi. Sağlıklı bireylerden oluşturulan kontrol grubu 8 erkek ve 7 kadın içermekte olup, yaş ortalaması 45.93±17.96 yıl (20-70) idi. Siroz hastalarından 6'sına daha öncesinde özofagus varislerine endoskopik iinjeksiyon skleroterapi uygulanmıştı. Bulgular: Hastaların 8'i Child A, 10'u Child B, 12'si Child C grubunda idi. Üreaz ve histoloji ile Helicobacter pylori varlığı siroz olgularının 12'sinde, kontrollerin ise 5'inde pozitif gösterildi. Child C grubunun gastrik mukoza hekzosamin düzeyleri Child A ve kontrol grubu ölçümlerinden istatistiki olarak anlamlı şekilde düşük idi (p 0.05). Sonuç: Özellikle ileri evre siroz olgularında gastrik mukozal savunmanın bozulduğu ve patofizyolojide Helicobacter pylori'ninde kendi başına rolü olduğu düşünüldü. Ek olarak; Helicobacter pylori'nin gastrik mukozal hekzosamin düzeylerini düşürerek gastrik mukozal hassasiyeti arttırabileceği düşünüldü.
