Abstract

Background/Aims: In spite of new drug regimens, the eradication of Helicobacter pylori infection remains problematic and factors interfering with therapeutic efficacy have yet to be clarified. In this study, the role of gastric histopathology in predicting resistant cases prior to therapy was investigated. Methods: A total of 74 dyspeptic patients with Helicobacter pylori infection (45 men, 29 women; mean age 40 years) were included in the study. Helicobacter pylori status was evaluated by histology and urease test at entry and four weeks after treatment. The Sydney system of gastric histology grading was applied to specimens taken from both the antrum and corpus to evaluate the severity of Helicobacter pylori -related gastritis. Eradication of the infection was defined as negative results of both tests. All patients received a two-week course of standard triple therapy (500 mg tetracycline qid and 750 mg metronidazole tid for two weeks and 300 mg bismuth subcitrate qid for four weeks). Results: The overall eradication rate was 59.5%. The eradication rate was different between patients with high, intermediate or low pretreatment gastritis activity (33%, 69 % and 58.8% respectively). The eradication rate of Helicobacter pylori in patients with lymphoid follicles in the antrum and corpus was similar to those without lymphoid follicles (63.6% vs 56.1%, p>0.05 and 57.8% vs 60%, p>0.05). Conclusion: This study suggests that cases of severe antral gastritis are resistant to eradication. Pretreatment histopathology may denote the success of eradication, determine the choice of regimen and predict cases of failed eradication.

Özet

Amaç: Yeni geliştirilen tedavi rejimlerine rağmen hala Helicobacter pylori enfeksiyonunu eradike etmek güçtür. Tedavi başarısını etkileyen faktörler tam belirlenememiştir. Ve biz bu çalışmada gastrik histolojinin tedaviye resistan olguların tedavi öncesi saptanmasındaki rolünü araştırmak istedik. Yöntem: Çalışmaya dispeptik yakınmaları olan Helicobacter pylori pozitif 74 (45'i erkek ve 29'u kadın, yaş ortalaması 40 yıl) olgu alındı. Helicobacter pylori varlığı hem çalışma başlangıcında hem de tedavi sonrası 4. haftada CLO testi ve histolojik olarak değerlendirildi. Antrum ve korpustan alınan patoloji spesmenlerindeki gastrit şiddeti Sydney sistemi ile değerlendirildi. Eradikasyon, her iki testinde negatifleşmesi ile pozitif olarak kabul edildi. Tüm olgulara 2 haftalık standart üçlü tedavi (Tetrasiklin 4X500 mg, Metranidazol 3X250 mg iki hafta ve bismuth subsitrat 4X300 mg 4 hafta) verildi. Bulgular: Eradikasyon oranı %59.5 idi. Eradikasyon oranı tedavi öncesi şiddetli, orta ve hafif gastritli olgularda istatiksel anlamlılığa ulaşılamamakla birlikte belirgin farklılık göstermekteydi (sırasıyla %33, %69, %58.8(p>0.05). Lenfoid follikül olan ve olmayan olgular arasında eradikasyon oranlarında farklılık yoktu. Sonuç: Bizim çalışmamız antral gastriti şiddetli olguların eradikasyona rezistan olgular olduğunu göstermektedir. Tedavi öncesi histolojik değerlendirme eradikasyon başarısının düşük olacağı olguları önceden saptamada etkin olabilir ve bu olgulara uygun rejimlerin seçilmesinde bize rehberlik edebilir.
