The efficacy of somatostatin treatment in bleeding oesophageal varices

Abstract

The aim of this study was to evaluate the ef¬ficacy of somatostatin (Stilamin; Serono) in the control of bleeding oesaphageal varices. Twenty four patients with acute variceal haemorrhage were included. Baloon tamponade was the first method in managing the bleed¬ing. Somatostatin was given to 11 patients (2 females and 9 males) who refused or resisted to tamponade. Somatostatin was given as a 250 /jg bolus followed by 250 fig I hour infusion for five days. The bleeding stopped in 5 patients (45.5%) in the study group (23.1% in the controls). The death rate was 27.3% in the study group and 38.4% in the controls. Endoscopic sclerother-apy was successfully performed to 3 patients in the study group while 2 patients were treated surgically in the controls. Somatostatin was found effective in man¬aging bleeding from esophageal varices in order to stop the bleeding, performing endoscopic sclerotherapy, and also may be effective in the transportation of the patient to a Gaströenterology clinic.

Özet

Çalışmamızın amacı özefagus varis kanamaları¬nı durdurmada somatostatinin etkilerini araştırmaktı. Çalışmaya akut kanamalı 24 hasta alındı. Kanamayı durdurmada ilk yöntem balon tamponadıydı. Somatos¬tatin tedavisi balon tamponadı uygulanamayan 11 has¬taya verildi. Somatostatin 250 \ıg bolusu takiben 250 fig I saat İV infüzyon tarzında beş gün süreyle verildi. Beş hastada kanama durdu (%45.5) (kontrol grubunda %23.1). Çalışma grubunda ölüm oranı %27.3, kontrol grubunda ise %38.4 idi. Çalışma grubunda 3 hastaya endoscopic skleroterapi başarılı şekilde uygulandı. Kontrol grubunda 2 hasta cerrahi olarak tedavi edildi. Somatostatinin özefagus varis kanamalarının tedavi¬sinde, endoskopik skleroterapide etkili olduğu ve hasta¬nın bir Gastroenteroloji kliniğine nakli esnasında etkili olabileceği saptanmıştır.
