Intrafamilial transmission of hepatitis C virus infection

Abstract
In this study, the intrafamilial transmission of hepatitis C virus (HCV) was investigated. For this aim, the serum samples from 496 family members, con¬sisting of 105 spouses, 246 children, 43 sublings and 102 other relatives and 140 index patients with chronic liver disease caused by HCV were screened for anti HCV anti¬bodies by second generation ELISA test. The serum samples were also analyzed for HCV RNA by PCR met¬hod. Control group comprised 324 healthy pregnant wo¬men. Anti HCV seroprevalence was found to be 4.2% in the family members of index patients, whereas, 0.3% in control group (p<0.01). The anti HCV prevalences in spo¬uses, children, siblings and other contacts of the index patients were 7.6%, 3.6%, 2.3% and 2.9% respectively. Anti HCV pozitivity was found in 21 of family members of HCV RNA positive 108 index cases where as anti HCV positivity was not detected in any of family member of HCV RNA negative 32 index cases. The results of this study suggest that the intrafamilial transmission of HCV primarily occurs by sexual transmission and the HCV RNA of the index patients being pozitive is impor¬tant for the spread of HCV.

Bu çalışmada hepatit C virusu (HCV) 'nun aile içi geçişi araştırıldı. Bu amaçla HCV etyolojili kronik kara-, ciğer hastalığı nedeniyle izlenen 140 indeks hasta ile bu hastaların yakınları olan 105 eş, 246 çocuk, 43 kardeş ve 102 diğer yakınlar olmak üzere toplam 496 aile yakını¬na ait serum örneklerinde anti HCV antikorları ikinci kuşak ELISA testi kullanılarak tayin edildi. Serum ör¬nekleri HCV RNA yönünden de PCR metodu ile araştı¬rıldı. Kontrol grubu 324 sağlıklı hamile kadından oluş¬maktaydı. İndeks olguların aile yakınlarındaki anti HCV seroprevalansı %4.2 olarak bulunurken kontrol grubunda ise %0.3 olarak saptandı (p<0.01). İndeks ol¬guların eş, çocuk, kardeş ve diğer yakınlarındaki anti HCV seropozitifliği sırasıyla %7.6, %3.6, %2.3 ve %2.9 olarak bulundu. HCV RNA pozitif 108 indeks olgunun aile yakınlarının 21'inde anti HCV pozitif bulunurken, HCV RNA negatif 32 indeks olgunun aile yakınlarının hiç birisinde anti HCV pozitifliği saptanmadı. Bu çalış¬manın sonuçları HCV'nin aile içi geçişinin öncelikle sek¬süel yol ile gerçekleştiğini ve indeks olgunun HCV RNA-'sının pozitif olmasının HCV'nin bulaşmasında önemli olduğunu göstermektedir.
