Abstract

Background/Aims: This retrospective study was carried out to observe the efficacy and safety of papillotomy with a needle-knife papillotomy when biliary access could not be achieved by the standard method in cases undergoing endoscopic retrograde cholangiopancreatography (ERCP). Methods: The needleknife papillotomy was used on 52 patients. Inability of an experienced endoscopist to achieve opacification or cannulation of the common bile duct by the standard method within 15 minutes was accepted as an indication for use of instrument. Results: In the first session, bile duct cannulation was achieved in 39 out of 52 patients (75%). Three to seven days following the first procedure, it was established in nine of the other 13 patients (17%). Thus biliary access was successful in 48 cases overall (92%). Percutaneous procedure was performed on two of the remaining cases and surgery on the other two. Perforation was the only complication, which occurred in three patients (5.7%), but only one of them required surgical treatment. Conclusion: These results suggest that in selected cases, papillotomy with the needleknife papillotome is a safe and quite efficient method of treatment which should be considered before a percutaneous procedure and surgical operation.

Özet

Amaç: Bu retrospektif çalışma, endoskopik retrograd kolanjiopankreatografi (ERKP) uygulaması esnasında standart yöntemle safra yoluna girilemeyen olgularda iğne uçlu papillotomla yapılan papillotominin etkinlik ve emniyetini araştırmak amacıyla yapıldı. Yöntem: İğne uçlu papillotomi 52 hastada uygulandı. Tecrübeli bir endoskopist tarafından ana safra kanalının kanüle edilememesi veya görüntülenememesi, iğne uçlu papillotom için endikasyon olarak kabul edildi. İlk işlemde 52 hastanin 39'unda (%75) safra kanalı kanülasyonu gerçekleştirilebildi. Bulgular: İlk girişimden 3-7 gün sonra diğer 13 hastanın 9'unda (%17) kanülasyon sağlandı. Böylece toplam 48 hastada (%92) biliyer kanülasyon sağlandı. Geri kalan dört hastanın 2'sinde perkütan girişim, 2'sinde cerrahi girişim uygulandı. Komplikasyon olarak sadece 3 hastada (%5.7) perforasyon gelişmiş olup bunların da yalnızca birinde cerrahi müdahale gerekli oldu. Sonuç: Seçilmiş olgularda iğne uçlu ile papillotomi, perkütan yol ve cerrahi operasyondan önce düşünülmesi gerekli, emniyetli ve oldukça etkili bir yöntem gibi görünmektedir.
