Aberan safra kanallarının klinik ve cerrahi önemi

Abstract
The great majority of injuries to the bile duct occur during cholecystectomy. The incidence of biliary injury is approximately 1 per 1000 open cholecystectomy (OC), while in laparoscopic cholecystectomy (LC), the reported risk of bile duct injury appears to be higher, at around 1 per 500 operations. A number of factors are related to bile duct injuries, one of which is anatomical variation consisting of right aberrant divisional bile ducts and accessory bile ducts (Luschka Channels). The aim of this study was to clarify the role of aberrant bile ducts in bile duct injury at LC. Four cases of right aberrant divisional bile ducts are described. Three cases were admitted to the hospital with bile leakage and cholangitis following cholecystectomy, uhile one case had cholangitis with no history of surgery. The first patient presented with cholangitis following inadvertant dissection of the bile duct during surgery, which had then been sutured on recognition. The second and fourth cases presented with bilioma and bilioma with fistula respectively, resulting from bile leakage from the duct, while the third patient was referred with cholangitis where the cystic duct opened to the disunited right hepatic duct due to gallstones fulling into the duct. Diagnosis was established by ultrasonography and endoscopic retrograde cholongiopancreatography with the addition of percutaneous transhepatic cholongiography (PTC) in the first two cases and fistulography in the fourth case. The first and the second cases were treated conservatively, the third case was treated by endoscopic sphincterotomy and balloon extraction and the fourth case surgically. Recognition of aberrant and accessory bile ducts by the endoscopist, surgeon and radiologist contributes to the prevention of bile duct injuries with a high morbidity and mortality rate.

Safra yolu hasarlarının çoğu kolesistektomi sırasında ortaya çıkar. Açık kolesistektomide (AK) biliyer sistem hasar insidansı yaklaşık 1/1000'dir. Laparoskopik kolesistektomide (LK) safra yolu yaralanmaları 1/500'e kadar bildirilmektedir. Çeşitli faktörler safra yolu hasarı ile ilişkilidir. Sağ aberan birleşmemiş safra yolları ve aksesuar kanalları içeren anatomik varyasyonlar (AV) bunlardan biridir. Bu çalışmanın amacı LK'deki safra yolu hasarlanmasında AV'ın önemini vurgulamaktır. Sağ aberan birleşmemiş safra yolları olan 4 hasta tanımlandı. Üç hasta kolesistektomiyi takiben kolanjit ve safra kaçağı ile, 1 hasta operasyon olmaksızın kolanjitle hastaneye kabul edildi. Birinci olguda birleşmemiş kanal LK sırasında kesilmiş, aynı seansta farkedilip dikilmiş ve takiben kolanjit gelişmişti. İkinci olgu kesilen kanaldan sızan safranın oluşturduğu bilioma ile, 4. olgu bilioma ve fistül ile başvurdu. Sistik kanalı, sağ birleşmemiş hepatik kanala açılan 3. olgu ise SK taşının bu kanala düşmesi sonucu kolanjit ile başvurmuştu. Tanı ultrasonografi, ERCP, ilk olguda ilave PTK ve 4. olguda ilave fistülografi ile kondu. Birinci ve 2. olgular konservatif tedavi ile, 4. olgu cerrahi ile ve 3. olgu ise endoskopik biliyer sfinkterotomi ve balon ekstraksiyonu ile tedavi edildi. Safra yolu yaralanmalarının endoskopist, cerrah ve rad-yologlar tarafından iyi tanınması, morbidite ve mortalitesi yüksek iatrojenik safra yolları yaralanmalarının önlenmesine katkıda bulunacaktır.
