Recompense child C sirozlu hastalarda özofagus varis kanaması kontrolünde terlipressin ve skleroterapi ile somatostatin ve skleroterapi kombinasyon tedavilerinin karşılaştırılması

Abstract
Background/Aims: Bleeding from esophageal varices is one of the most dramatic complications of portal hypertension. It has recently been established that the combination of vaso-active drugs with sclerotherapy is effective on the initial control of bleeding from esophageal varices. The aim of this study was to compare the efficacy of somatostatin plus sclerotherapy with that of terlipressin plus sclerotherapy on initial control of variceal bleeding in patients with cirrhosis. Methods: Thirty four patients (20 male, 14 female, mean age 56.7±12 years) with variceal bleeding found on endoscopic examination were randomised into two groups. They were given either a 250mg bolus injection of somatostatin followed by a 250mg per hour infusion or a bolus injection of terlipressin 2mg followed by a 2mg intravenous infusion every four hours. Patients in both groups underwent sclerotherapy within half an hour of vasoactive drug administration. Results: Fifteen patients were found to have hepatitis B virus, sixteen hepatitis C virus and three excessive alcohol consumption on etiological investigation. All patients had cirrhosis of Child C score. Initial control of bleeding was achived in twelve patients prescribed somatostatin plus sclerotherapy and fourteen patients prescribed terlipressin plus sclerotherapy within 48 hours of admission. Three patients in the somatostatin plus sclerotherapy group died within two days of bleeding and two patients within three days of bleeding (21%). Three patients in the terlipressin plus sclerotherapy group (11%) died within three days of bleeding. Conclusions: That somatostatin plus sclerotherapy and terlipressin plus sclerotherapy was found to have a similar spectrum of activity in initial control of bleeding from esophageal varices due to cirrhosis.

Amaç: Özofagus varis kanaması portal hipertansiyonun en dramatik komplikasyonlarından birisidir. Son zamanlarda vazoaktif ilaçlarla skleroterapinin birlikte kullanımının, özofagus varis kanamalarının başlangıç kontrolünü sağlamada etkili olduğu bildirilmektedir. Amacımız sirozlu hastalarda, somatostatin+skleroterapi ve terlipressin+skleroterapi'nin, varis kanamalarının başlangıç kontrolunün sağlanmasındaki etkinliklerini karşılaştırmaktır. Yöntem: Endoskopik inceleme ile varis kanamaları tespit edilen 34 olgu çalışma kapsamına alındı. Randomize edilen hastalara terlipressin 2 mg bolus daha sonra 4 saatte bir 2 mg intravenöz yada somatostatin 250 µg bolus ve 250 µg/h gidecek şekilde infüzyon uygulandı. Her iki gruptaki hastalara vazoaktif ilaç uygulanmasından en geç yarım saat içinde skleroterapi uygulandı. Bulgular: Olguların 20'si erkek ve 14'u kadın olup ortalama yaşları 56.7±12 yıl idi. Etyolojik ajan olarak olguların 15'inde hepatit B virüsü, 16'sında hepatit C virüsü ve üçünde alkol tespit edildi. Olguların tamamı Child C grubuna dahildi. Somatostatin+ Skleroterapi grubundaki hastaların 12'sinde, terlipressin+ skleroterapi grubundaki olguların 14'ünde ilk 48 saatte içinde kanamanın başlangıç kontrolü sağlandı. Somatostatin+ skleroterapi grubunda üç olgu kanama başlangıcının ikinci gününde ve iki olgu üçüncü gününde (%21), terlipressin+ skleroterapi grubunda ise üç olgu (%11) kanamanın üçüncü gününde kaybedildi. Sonuç: Siroza bağlı gelişen özofagus varis kanamalarının başlangıç kontrolunün sağlanmasında, somatostatin+skleroterapi ile terlipressin+skleroterapi'nin etkilerinin benzer olduğu kanaatine varıldı.
