Does Helicobacter pylori eradication improve the symptoms of globus hystericus?

Abstract

Background/aims: Globus hystericus is a feeling of a tension in the throat, irrelevant of swallowing, persisting for at least 12 we- eks. Since the cause of globus hystericus is not fully described, a controversy exists about the treatment options. We aimed in this study to determine the symptoms of gastroesophageal reflux disease, the upper gastrointestinal endoscopic findings, prevalence of Helicobacter pylori, and the post-treatment symptoms (symptoms of gastroesophageal reflux disease and/or Helicobacter pylori) in patients with a diagnosis of globus hystericus. Materials and Methods: In this study, we retrostectively evaluated the medical re- cords of 123 patients who, after the initial diagnosis of globus sensation (globus hystericus), underwent endoscopic examination in the Department of Gastroenterology and Endoscopy at Celal Bayar University Medical School, and thereafter, were given Helico- bacter pylori eradication therapy and were followed in the Gastroenterology outpatient clinic, between January 2009 and August 2010. Results: Helicobacter pylori was positive in seventy-five (60%) of the 123 patients. Heartburn, regurgitation, and inlet patch in the upper esophagus were significantly more frequent in Helicobacter pylori (+) patients than in Helicobacter pylori (-) patients with globus hystericus. Normal endoscopy findings were significantly more common in Helicobacter pylori (-) patients than in He- licobacter pylori (+) patients with globus hystericus. Of patients with eradicated Helicobacter pylori, 27 (50%) reported that globus symptoms had disappeared, while 12 (17.3%) of them had not observed any regression of the symptoms. Improvement of symptoms had a positive correlation with Helicobacter pylori eradication (p=0,001). Conclusions: Helicobacter pylori prevalence among ca- ses with globus sensation is similar to that in the general population. However, Helicobacter pylori eradication was found to dec- rease the symptoms of globus.

Girifl ve Amaç: Globus histerikus en az 12 haftadır devam eden boğazda takılma hissi, yutmada zorlanma olarak tanımlanmak- tadır. Globus histerikus sebebi net olarak açıklanmadığı gibi tedavisinde de bir fikir birliği yoktur. Bu çalıflmada amacımız endos- kopi ünitesine globus histerikus ön tanısı ile sevk edilen ve üst gastrointestinal endoskopisi yapılan olgularda; gastroözofageal ref- lü semptomları, üst gastrointestinal endoskopik bulguları, Helikobakter pilori prevelansını saptamak ve Helikobakter pilori eradi- kasyonu yapılan olgularda globus histerikus semptomlarının durumunu belirlemektir. Gereç ve Yöntem: Ocak 2009-Ağustos 2010 yılları arasında Celal Bayar Üniversitesi Tıp Fakültesi Gastroenteroloji Kliniğinde boğazda takılma hissi (Globus Histerikus?) ön tanısı ile endoskopi yapılan ve Gastroenteroloji polikliniğinde eradikasyon tedavisi verilen ve tekrar kontrole gelen 123 olgunun dos- ya bilgileri retrospektif olarak kaydedildi. Bulgular: Globus histerikuslu 123 olgunun 75'inde Helikobakter pilori (+) olarak bu- lunmufltur. Helikobakter pilori (+) olgularda göğüs arkasında yanma, ağza acı, ekfli su gelme ve üst özofagus düzeyinde inlet patch Helikobakter pilori (-) olgulara gore anlamlı olarak daha yüksek bulundu. Helikobakter pilori (-) olgularda normal endoskopi Helikobakter pilori (+) olgulara göre anlamlı olarak daha yüksek idi. Helikobakter pilorisi eradike olan 27 (%50) olguda globus his- terikus semptomlarının geçtiği 12 (%17.3) kiflide ise herhangi bir değifliklik olmadığı bulunmufltur. Helikobakter pilori eradikasyo- nu ile semptomların gerilemesinde pozitif korelasyon bulunmufltur. Sonuç: Helikobakter pilori globus histerikus olan olgularda toplumda aynı düzeyde olmakla birlikte Helikobakter pilori tedavisinin globus histerikus flikayetlerini gerilettiği görülmüştür.
