Could the Alma-Ata Declaration Affect the Health Systems of the Countries

Abstract
This article aimed to evaluate the possible effects of the International Conference on Primary Health Care (PHC) held in Alma-Ata in 1978 on the health systems of countries. Since the beginning of the 1900s, an understanding of egalitarian and comprehensive health care has emerged in different parts of the world within the scope of rights movements, and the accumulation gained within the framework of this und erstanding has been the pioneer of the Alma-Ata Declaration. The Selective Health Service approach and global initiatives, which could be seen as the anti- thesis of comprehensive PHC, have been developed within a few years without time for the declaration to have an impact on countries’ health systems.InthefollowingperiodneoliberalhealthreformswerecarriedoutbytheWorldBank.Thechangeinhealthsystemsfollowing the Alma- Ata Declaration was evaluated through examples from two Asian countries, India and Sri Lanka, Sub-Saharan Africa countries, Latin American countries, and Canada. The conclusion was reached that the impact of the Alma-Ata Declaration on health care systems is limited only to certain year periods, communities or regions and in an environment where there is no objection to capitalist modes of production and neoliberal health policies.Therefore,itisnotpossibleforanymanifestotohaveaneffectthatmaintainsanegalitarian,democratic,community-inclusive,inclusive andholistichealthsystem.

Bu yazıda, 1978 yılında Alma-Ata’da gerçekleşen UluslararasıTemel Sağlık Hizmetleri (TSH) Konferansı’nın ülkelerin sağlık sistemleri üzerin- deki olası etkilerinin değerlendirilmesi amaçlanmıştır. Dünyanın farklı bölgelerinde 1900’lü yılların başından bu yana verilen hak mücade- leleri kapsamında eşitlikçi ve kapsamlı sağlık hizmeti anlayışı doğmuş ve bu anlayış çerçevesinde elde edilen birikim Alma-Ata Bildirgesi’nin öncüsü olmuştur. Bildirgenin ülkelerin sağlık sistemleri üzerinde bir etki yaratmasına zaman kalmadan birkaç yıl içinde kapsaml ı TSH’nin anti-tezi olarak görülebilecek Seçici Sağlık Hizmeti yaklaşımı ve küresel girişimler geliştirilmiş, ilerleyen süreçte Dünya Bankası(DB) eliyle neoliberal sağlık reformları yürütülmüştür. Bu makalede, Alma-Ata Bildirgesi’nin ardından sağlık sistemlerinin nasıl bir değişim gösterdiği Hindistan ve Sri Lanka başta olmak üzere iki Asya ülkesi, Sahraaltı Afrika ve Latin Amerika ülkeleri ile Kanada örnekleri aracılığıyla değerlen- dirilmiştir.Bildirgeninsadecebelliyıllarla,topluluklarlayadabölgelerlesınırlıkalanetkilerininolduğu,kapitalistüretimbiçimlerineve neoli- beralsağlıkpolitikalarınatamamenkarşıçıkılmayanbirortamdaherhangibirbildirgenineşitlikçi,demokratik,toplumkatılımlı,kapsayıcıve bütüncül bir sağlık sisteminin sürdürülebilmesini sağlayan bir etki göstermesinin mümkün olamayacağı sonucuna varılmıştır
