MICROPAPILLARY TYPE ADENOCARCINOMA PRESENTING AS A BILATERAL MULTILOBAR NON-RESOLVING CONSOLIDATION FOR TWO YEARS

Abstract
Radiograph in adenocarcinoma can rarely demonstrate bilateral multilobar consolidation, often with air bronchograms that are indistinguishable from pneumonia. We present a case of a 65-year-old female who was evaluated for a bilateral multilobar consolidation for a period of two years. She had undergone an inconclusive bronchoscopy and had received several courses of antibiotics including anti-tuberculous therapy without relief. The size of the lesion had remained largely unchanged during this period and there was no significant clinical deterioration in the patient. Thoracoscopic wedge resection from right upper, middle and lower lobes was performed on presentation, which revealed a histopathologic diagnosis of micropapillary type adenocarcinoma. We report case of adenocarcinoma presenting as a bilateral multilobar consolidation without significant change for a period of two years, which has rarely been documented in the literature.

Adenokarsinomada radyografi, pnömoniden ayırt edilemeyen ve hava bronkogramlarının sıklıkla birlikte olduğu bilateral multilobar konsolidasyonları nadiren gösterebilir. İki yıldır devam eden bilateral multilobar konsolidasyonlardan dolayı takip edilen 65 yaşındaki kadın hasta sunuldu. Hastada bronkoskopi tanısal değildi ve anti tüberküloz dahil bir çok defa antibiyotik tedavisi almasına rağmen iyileşme sağlanamamıştı. Bu periyotta hastanın lezyonlarında belirgin değişikliklergörülmedi ve kliniğinde anlamlı kötüleşme olmadı. Sağ üst, orta ve üst lobtan torakoskopik wedge rezeksiyon yapılan hastada mikropapiller adenokarsinomatanısı kondu. Çalışmamızda literatürde çok az sunulan, iki yıllık periyotta anlamlı değişikliklerin olmadığı bilateral multilobar konsolidasyonlarla ortaya çıkanadenokarsinoma olgusunu sunmayı amaçladık.
