Evaluation of the relationship between the season of birth and neurodevelopmental psychiatric disorders

Abstract
Objective: There may be an association with the season of birth in the etiology of neurodevelopmental psychiatric disorders in which infections and deficiency of vitamin D play roles on the pathogenezis. Although there are discrepancies between the results in the literature, data about this association for our country is limited. Therefore we aim to assess the relationship between the season of birth and the childhood neurodevelopmental disorders including attention deficit hyperactivity disorder (ADHD), tic disorders, obsessive compulsive disorder (OCD) and autism spectrum disorders (ASD). Methods: Medical charts of the cases who had admitted to the child and adolescent psychiatry outpatient clinic of the university hospital as a first referral in a one year duration, were assessed. The season of birth, birth timing, delivery type and the psychiatric diagnoses were evaluated retrospectively. Neurodevelopmental psychiatric group was composed of subjects with ADHD, ASD, OCD and tic disorders. Subjects in the control group were without any neurodevelopmental disorders and they had other diagnosis such as depression, anxiety, enuresis, encopresis andadaptational disorder. These two groups were compared for the season of birth. Results: Among total of 616 subjects; 29.2% (n=180) of them were girls and 70.8% (n=436) were boys. The most common seasons of birth were as following; summer (30.3%) for ADHD, winter (32.9%) for the ASD, spring (29%) for the tic disorders and winter (36.8%) for the OCD. There was no association between the diagnostic classes and the season of birth (p>0.05). Besides there was no association in the seasonality between neuropsychiatric disorders and control group (p>0.05). Conclusion: This study showed a neglectable association between the season of birth and the neurodevelopmental psychiatric disorders. Further studies on larger clinic population assesing the precipitating factors such as prenatal and perinatal risks, levels of vitamine D, maternal nutritional status are needed for the proper comparison of the impact of season of birth.

Amaç: Nörogelişimsel psikiyatrik hastalıkların etiyolojik nedenleri arasında doğum mevsiminin yer alabileceği ve D vitamini eksikliği ve enfeksiyonların patogenezde rol oynadığı ileri sürülmektedir. Mevcut verilerde tutarsızlıklar olmasına rağmen, ülkemizde doğum mevsimi ilişkisini inceleyen çalışmalar kısıtlıdır. Bu çalışmada, dikkat eksikliği hiperaktivite bozukluğu (DEHB), tik bozukluğu, obsesif kompulsif bozukluk (OKB) ve otizm spektrum bozukluğunu (OSB) içeren çocukluk çağı nörogelişimsel bozukluklar ile doğum mevsimi arasındaki ilişkinin araştırılması amaçlanmıştır. Gereç ve yöntem: Üniversite hastanesi çocuk ve ergen psikiyatrisi polikliniğine bir yıllık süreçte ilk kez başvuran olguların dosyaları değerlendirildi. Doğum mevsimleri, aldıkları psikiyatrik tanılar, doğum şekli ve zamanı geriye dönük olarak incelendi. Nörogelişimsel psikiyatrik bozukluk grubuna DEHB, OKB, OSB, tik bozukluğu tanısı alan olgular dahil edildi. Kontrol grubuna nörogelişimsel bozukluk sayılmayan anksiyete bozuklukları, depresyon, enürezis, enkoprezis ve uyum bozukluğu tanısı olan olgular alındı. Doğum mevsimi açısından gruplar karşılaştırıldı. Bulgular: Toplam 616 olgunun %29,2'si (n=180) kız, %70,8'i erkekti. DEHB tanısı alan olguların en fazla yazın (%30,3), OSB tanısı alanların kışın (%32,9), tik bozukluğu olanların ilkbahar (%29) ve OKB olgularının kış (%36,8) mevsiminde doğmuş oldukları belirlendi. Ancak tanılar ve mevsimler arasında istatistiksel olarak anlamlı fark yoktu (p>0.05). Nörogelişimsel bozukluklar ile kontrol grubu arasında da doğum mevsimleri arasında anlamlı bir ilişki bulunamadı (p>0.05). Sonuç: Çalışmamızda nörogelişimsel psikiyatrik hastalık tanıları ile doğum mevsimi arasında ilişki saptanmamıştır. Doğum mevsimselliğini kıyaslamak için daha büyük hasta gruplarını içeren ve etkili olabilecek prenatal ve perinatal riskler, D vitamini seviyesi, anne beslenme durumu gibi diğer karıştırıcı faktörlerin incelendiği daha kapsamlı araştırmalara ihtiyaç vardır.
