THE EVALUATION OF OBSTRUCTION DEGREE BY POSITION AND SLEEP DEPTH IN SLEEP ENDOSCOPY IN OBSTRUCTIVE SLEEP APNEA PATIENTS

OBSTRÜKTİF UYKU APNESİ HASTALARINDA UYKU ENDOSKOPİSİNDE POZİSYON VE UYKU DERİNLİĞİNE GÖRE OBSTRÜKSİYON DERECESİNİN DEĞERLENDİRİLMESİ

Abstract

Objective Anesthesia management in obstructive sleep apnea (OSA) patients is particularly important for airway management. In our study, it was aimed to evaluate the upper airway by determining obstruction sites endoscopically according to OSA patients’ sleep depth and position. Matherial and Methods The study included 47 patients who underwent polysomnography and were diagnosed with OSA. Patients were evaluated with drug induced sleep endoscopy (DISE) in preoperative period. With flexible nasopharyngoscope, superficial sedation at the supine and lateral position; and in the deep sedation at the supine and lateral position, the obstruction areas of the patients were evaluated according to VOTE classification. Results When the degree of obstruction during deep sedation at the lateral position was compared with superficial sedation, the difference was found statistically significant in vellum level (p = 0.006). Conclusion In our study, the obstruction under the deep sedation at the supine position was determined more common than in the superficial sedation and lateral position.

AmaçObstrüktif uyku apnesi (OSA) hastalarında anestezi yönetimi peroperatif dönemde özellikle havayolu yönetimi açısından özellik göstermektedir. Çalışmamızda OSA hastalarının uyku derinliğine ve pozisyona göre obstrüksiyon bölgeleri endoskopik olarak tespit edilerek üst havayolu ile ilgili değerlendirmeler yapılması amaçlanmıştır.Gereç ve YöntemÇalışmaya polisomnografi yapılmış ve OSA tanısı almış 47 hasta dahil edildi. Hastalar preoperatif dönemde uyku endoskopisi (UE) ile değerlendirildi. Fleksiblnazofaringoskop ile yüzeyel sedasyonda supin ve lateral pozisyonda, derin sedasyonda supin ve lateral pozisyonda hastaların obstrüksiyon bölgeleri VOTEsınıflamasına göre değerlendirildi.BulgularVellum düzeyinde, lateral pozisyonda derin sedasyon sırasındaki obstrüksiyon derecesi yüzeyel sedasyon ile karşılaştırıldığında aradaki farklılık istatistiksel olarak anlamlı tespit edilmiştir (p=0,006).SonuçÇalışmamızda supin pozisyonda derin sedasyon altındaki obstrüksiyon yüzeyel sedasyona göre ve lateral pozisyona göre daha fazla tespit edilmiştir.
