Abstract

In physiological and pathological conditions, two main cell deaths occur with different processes called nec rosis and apoptosis. It is stated that dysregulation in apoptosis triggers cancer or autoimmunity and exces sive apoptosis is associated with neurodegenerative diseases. The ways of cells escape apoptosis are being investigated for the treatment of cancer chara cterized by increased proliferation. In relation to this, it has been identified that antiapoptotic proteins such as Bcl-2, Bcl-xL, and Mcl-1 are increased, proapopto tic proteins are decreased in cancer cells. Among the many proteins and protein complexes involved in cell death, another important group is inhibitor of apopto sis (IAP) protein family. IAPs function as endogenous caspase inhibitors capable of suppressing both intrin sic and extrinsic pathways in apoptosis and are in volved in cell division and immune regulation besides apoptosis. The proteins that have been shown to be overexpressed, such as Bcl-2 and the members of the IAPs family, are useful in both the diagnostic and the therapeutic formalities. Recently, the drugs targeting only the cancer cells have entered into the treatment protocols. In this review, molecular mechanisms of apoptotic pathways and related new therapeutic ap proaches are discussed in a concise manner.

Özet

Fizyolojik ve patolojik durumlarda, işleyişleri farklı, nekroz ve apoptoz olmak üzere iki ana hücre ölü mü meydana gelir. Apoptoz basamaklarındaki disre gülasyonun, kanser veya otoimmüniteyi tetiklediği, aşırı apoptozun ise nörodejeneratif hastalıklarla iliş kilendirildiği çalışmalarda bildirilmektedir. Proliferas yon artışıyla karakterize edilen kanserin tedavisi için hücrelerin apoptozdan kaçış yolları araştırılmakta dır. Bununla ilişkili olarak kanser hücrelerinde Bcl-2, Bcl-xL ve Mcl-1 gibi antiapoptotik proteinlerin arttığı, proapoptotik proteinlerin ise azaldığı belirlenmiştir. Hücre ölümünde görev alan birçok protein ve protein kompleksleri arasında bir diğer önemli grubu apoptoz inhibitörü (IAP) protein ailesi oluşturmaktadır. IAP’lar apoptozda hem intrinsik hem de ekstrinsik yolağı bas kılayabilen endojen kaspaz inhibitörleri olarak fonksi yon görmekte olup, apoptoz dışında hücre bölünmesi ve immün regülasyonda da rol almaktadırlar. Bcl-2 ve IAP ailesi üyeleri gibi aşırı ekspresyonu tespit edilen proteinler hem tanı koyma hem de tedavi aşamasın da yarar sağlamaktadır. Günümüzde sadece kanser hücresini hedefleyen ilaçlar tedavi protokolleri arasına girmiş bulunmaktadır. Derlememizde apoptotik yolak lara ait moleküler mekanizmalar ve onlarla ilişkili he defe yönelik yeni tedavi yaklaşımları genel hatlarıyla irdelenmektedir.
