OUR ENDOVASCULAR STENT GRAFT (EVAR) EXPERIENCES IN ABDOMINAL AORTIC ANEURYSMS-SINGLE CENTER EXPERIENCE

Abstract

Objective In this study, we evaluated the early and mid-term results of 32 patients who underwent endovascular aortic aneurysm repair (EVAR) with diagnosis of abdominal aortic aneurysm. Materials and Methods A total of 32 patients who underwent endovascular aortic aneurysm repair procedure between January 2011 and September 2019 were included in this study. Demographic characteristics, perioperative and postoperative outcomes of patients have been evaluated. Results The mean age of patients was 71.4±8.4; 28 of them were male and 4 were female. Eight patients (%25) developed complications in the early postoperative period. Type 1 endoleak occurred in two patients, and graft thrombosis occurred in one patient. Femoral embolectomy was performed in one patient due to femoral artery thrombosis. Two patients demonstrated a fever of unknown origin. One patient was intubated due to respiratory distress in the early postoperative period and was followed up in the intensive care unit for two days. Gastrointestinal bleeding occurred in one patient four days after the procedure. An acute rupture occurred 49 months after the procedure for one patient, and open graft interposition was performed. No mortality was observed during the follow-up period. Conclusion Endovascular aortic aneurysm repair should be the preferred treatment modality for abdominal aortic aneurysms because it has lower surgical risk profile compared with open surgery. In these patients, strict radiological follow-up is crucial for detect postoperative complications and determining the requirement for a secondary intervention.

AmaçBu çalışmada abdominal aort anevrizması tanısı ileendovasküler aort anevrizması tamiri (EVAR) uyguladığınız 32 hastanın erken ve orta dönem sonuçlarıdeğerlendirildi.Gereç ve YöntemOcak 2011-Eylül 2019 tarihleri arasında endovasküleraort anevrizması onarım prosedürü uygulanan toplam32 hasta çalışmaya dahil edildi. Hastaların demografik özellikleri, perioperatif ve postoperatif sonuçlarıdeğerlendirildi..BulgularHastalarda ortalama yaş 71.4±8.4; 28 hasta erkek, 4hasta kadın idi. 8 hastada (%25) postoperatif erkendönemde komplikasyon gelişti. İki hastada Tip I endoleak, bir hastada greft trombozu meydana geldi.Bir hastada femoral arter trombozu nedeni ile femoralembolektomi yapıldı. İki hastada nedeni bilinmeyenateş gelişti. Bir hasta erken dönemde solunum sıkıntısı nedeni ile entübe edildi ve iki gün yoğun bakımdaizlendi. Bir hastada işlemden dört sonra gastrointestinal kanama görüldü. Bir hastada işlemden 49 aysonra akut rüptür gelişti ve açık greft interpozisyonuyapıldı. Takip süresi boyunca mortalite gözlenmedi.SonuçAbdominal aort anevrizmalarında endovasküler aortanevrizması tamiri açık cerrahiye kıyasla düşük riskprofiline sahip olması nedeni ile tercih edilmesi gereken bir tedavi yöntemidir. Bu hastalarda ameliyatsonrası komplikasyonları tespit etmek ve sekondermüdahale gereksinimini belirlemek için sıkı radyolojiktakip önemlidir.
