Mycosis fungoides'te tüm cilt elektron tedavisi ve tedavi morbiditesinin değerlendirilmesi

Abstract
The purpose of this study is to report in vivo dosimetric measurements, clinical results and late sequelae of the total skin electron beam therapy (TSEBT). Between March 1997 and March 2004, 18 mycosis fungoides patients have been treated at Gulhane Military Medical School, Department of Radiation Oncology. Patients with T2-3N0M0B0 disease have received 28Gy TSEBT in 7 fractions. The median follow-up time was 20.5 months from the completion of TSEBT. At 3 months after TSEBT, complete response was established in 14 patients (77.8%) and partial response in 4 patients (22.2%). Late skin injury parameters of atrophy, pigmentation change, hair loss, telangiectasia, and ulceration evaluated according to RTOG/EORTC late skin injury scale were grade 1 in 3 patients (16.7%), grade 2 in 8 patients in (44.4%), grade 3 in 6 patients (33.3%) and grade 4 in 1 patient(5.6%). In conclusion, we can say that the modified translational technique is an easy technique capable of covering the transverse dimensions of the patients resulting in early remission of the lesions and lowering of the underdosage areas, however the high fraction size we used is probably the main reason of increased late skin morbidity, although more deeply situated tissues are spared.
Özet
Bu çalışmanın amacı Tüm Cilt Elektron Tedavisinin (TCET) in vivo dozimetrik ölçümleri, klinik sonuçları ve geç sekelini sunmaktır. Mart 1997 ve Mart 2004 tarihleri arasında Gülhane Askeri Tıp Akademisi Radyasyon Onkolojisi departmanında 18 mycosis fungoides hastası tedavi edilmiştir. T2-3 N0 M0 B0 hastalıklı hastalara 7 fraksiyonda 28 Gy TCET uygulanmıştır. TCET'nin tamamlanmasından itibaren ortalama takip zamanı 20,5 aydır. TCET sonrası 3 ayda 14 hastada (%77,8) tam yanıt, 4 hastada (%22,2) parsiyel yanıt sağlanmıştır. Geç cilt radyasyon hasar parametreleri olan atrofi, pigmentasyon değişikliği, epilasyon, telenjiektazi ve ülserasyon RTOG/EORTC geç cilt hasar skalasına göre yapılan gradlamada 3 hastada grad 1 (%16,7), 8 hastada grad 2 (%44,4), 6 hastada grad 3 (%33,3) ve 1 hastada grad 4 geç cilt sekeli saptanmıştır. Sonuç olarak kullandığımız modifiye translasyonel, tekniğin lezyonların erken remisyonu ile sonuçlanıp az doz alanlarını azaltıp, hastaların transvers boyutlarını kapsayabilen kolay bir tekniktir, ancak daha derin yerleşimli dokular, korunmasına karşın kullandığımız yüksek fraksiyon dozu artmış geç cilt morbiditesinin muhtemel ana sebebidir.
