EVALUATION OF TOXOPLASMA GONDII SEROPREVALENCE IN DIFFERENT AGE GROUPS AND IGG AVIDITY TEST RESULTS IN ISPARTA PROVINCE

Abstract
Objective Toxoplasmosis is a common parasitic infectious disease worldwide caused by Toxoplasma gondii. The aim of this study was to investigate the seroprevalence of T.gondii in different age groups in our region and to evaluate IgG avidity test results. Material and Methods The serological results of the patients who were admitted to our hospital between January 2014-December 2018 and who were investigated for both anti-T. gondii IgM and anti-T.gondii IgG tests were evaluated retrospectively. Anti-T.gondii IgM/IgG tests and IgG avidity test were performed with chemiluminescent immunoassay method (Cobas e601, Roche Diagnostics, Germany) and enzyme-linked fluorescent assay method (VIDAS, bioMerieux, France), respectively. Results Of the 5454 patients included in the study, anti-T.gondii IgM and IgG seropositivity rates were found to be 3.8% and 25.5%, respectively. IgM and IgG seropositivity rates were 4.7% and 26.8% in women (n=3839) and 1.6% and 22.4% in men (n=1615), respectively. IgG seropositivity was found to be the lowest in the 0-10 age group (11.1%) and highest in the >60 age group (55.4%). Avidity test was requested from 151 cases with both IgG and IgM positive, and 125 cases with positive IgG and negative IgM. Of the 276 cases tested for IgG avidity test, 43 (15.6%) had low avidity, 18 (6.5%) had borderline avidity, and 215 (77.9%) had high avidity. Approximately two-thirds (66.2%) of the patients with both IgG and IgM positive had high avidity and it was considered to indicate a past infection. Discussion It was determined that anti-T.gondii IgG seropositivity rate was 25.5% in our region and IgG seropositivity rate increased with age. In cases where both anti-T. gondii IgM and IgG are positive, it is important to differentiate between acute infection and past infection by using IgG avidity test for accurate diagnosis and treatment.

AmaçToxoplasmosis, Toxoplasma gondii’nin neden olduğutüm dünyada yaygın olarak görülen bir paraziter enfeksiyonhastalığıdır. Bu çalışmada bölgemizde farklıyaş gruplarında T.gondii seroprevalansının araştırılmasıve IgG avidite test sonuçlarının değerlendirilmesiamaçlanmıştır.Gereç ve YöntemOcak 2014-Aralık 2018 tarihleri arasında hastanemizebaşvuran anti-T.gondii IgM ve anti-T.gondii IgG testlerieşzamanlı istenilen hastaların serolojik sonuçlarıretrospektif olarak incelendi. Anti-T.gondii IgM ve IgGtestleri chemiluminescent immunoassay yöntemiyle(Cobas e601, Roche Diagnostics, Almanya), IgG aviditetesti enzyme-linked fluorescent assay yöntemiyle(VIDAS, bioMerieux, Fransa) çalışıldı.BulgularÇalışmaya alınan toplam 5454 hastada anti-T.gondiiIgM ve IgG seropozitiflik oranları sırasıyla %3.8ve %25.5 olarak bulunmuştur. IgM ve IgG seropozitiflikoranları, kadınlarda (n=3839) sırasıyla %4.7 ve%26.8, erkeklerde (n=1615) ise %1.6 ve %22.4 olarakbelirlenmiştir. IgG seropozitifliği en düşük 0-10yaş grubunda (%11.1), en yüksek >60 yaş grubunda(%55.4) saptanmıştır. IgG’si ve IgM’si birlikte pozitifolan 151 olguda ve IgG’si pozitif, IgM’si negatif olan125 olguda avidite testi istenmiştir. IgG avidite testiuygulanan toplam 276 olgunun 43’ünde (%15.6) düşükavidite, 18’inde (%6.5) sınırda avidite ve 215’inde(%77.9) yüksek avidite saptanmıştır. IgG’si ve IgM’sibirlikte pozitif olan olguların yaklaşık üçte ikisinde(%66.2) yüksek avidite saptanmış ve önceden geçirilmişenfeksiyon olarak değerlendirilmiştir.SonuçBölgemizde, anti-T.gondii IgG seropozitiflik oranı%25.5 olarak belirlenmiş olup IgG seropozitiflik oranınınyaşla birlikte arttığı görülmüştür. Anti-T.gondii IgMve IgG’nin birlikte pozitif olduğu olgularda, IgG aviditetestiyle akut enfeksiyon ile önceden geçirilmiş enfeksiyonunayırımının yapılması doğru tanı ve tedavi açısındanönemlidir.
