Complications of surgical treatment in congenital pes equinovarus

Abstract
This study was carried out so as to evaluate the complications encountered in cases of congenital pes equinovarus which were treated surgically in our clinic. The treatment of congenital pes equinovarus has been performed surgically after the third month. Surgical interventions applied were posterior release, posteromedial release and complete subtalar release according to the severity of deformity. Various complications due to the techniques themselves or erroneous application of the techniques could take place. The results and complications of 46 feet of 32 patients who were operated in the Department of Orthopaedics and Traumatology in Haseki Educational and Research Hospital of Ministry of Health between the dates of May 1983 and July 1997, and came to the controls by accepting our invitations were evaluated retrospectively. Complications most frequently encountered clinically were forefoot adduction in 24 feet, heel valgus in 8 feet, sagging of medial longitudinal arch in 6 feet, postoperative wound dehiscence in 6 feet. On the other hand, complications which were recognized radiologi-cally were metatarsus varus in 18 feet, talonavicular subluxation in 9 feet, calcaneocuboidal subluxation in 6 feet and cutting of sustentaculum tali in 3 feet. The aim. of the treatment in congenital pes equinovarus is to obtain a functional, painfree, mobile and plantigrade foot which is able to wear a normal shoe. Open reduction of talocalcaneon-avicular joint in congenital pes equinovarus is very delicate surgical procedure. Postoperative follow-up is as important as the surgery. Surgical interventions should be peformed by experienced surgeons and the patients should be followed up in the postoperative period so as to minimize the rates of complications.

Amaç: Bu çalışma, kliniğimizde cerrahi olarak tedavi edilen doğuştan çarpık ayak deformiteli olgularda karşılaştığımız komplikasyonları irdelemek amacıyla yapılmıştır. Durum Değerlendirilmesi: Doğuştan çarpık ayağın tedavisi 3. aydan sonra cerrahi olarak yapılmaktadır. Deformitenin ciddiyetirıe göre uygulanan cerrahi işlemler posterior gevşetme, posteromedial gevşetme ve tam subtalar gevşetmedir. Bu tekniklerin kendisinden veya hatalı uygulanmasından kaynaklanan komplikasyonlar meydana gelebilmektedir. Hastalar ve Yöntem: Sağlık Bakanlığı Haseki Eğitim ve Araştırma Hastanesi Ortopedi ve Travmatoloji Kliniğinde Mayıs 1983-Temmuz 1997 tarihleri arasında opere edilen ve çağrımıza uyarak kontrole gelen 32 olgunun 46 ayağı sonuçları ve komplikasyonları yönünden retrospektif olarak değerlendirildi. Bulgular: Klinik olarak en sık karşılaşılan komplikasyonlar; 24 ayakta ayak önü adduksiyonu, 8 ayakta topuk valgusu, 6 ayakta medial longitudinal ark çökmesi, 6 ayakta postoperatif yara dudaklarında açılma gözlenirken, radyolojik olarak ise; 18 ayakta metatarsus varus, 9 ayakta talonaviküler subluksasyon, 6 ayakta kalkaneoküboid subluksasyon, 3 ayakta ise sustentakulum talinin kesilmesi gibi komplikasyonlar belirlendi. Sonuç: Doğuştan çarpık ayakta tedavinin amacı fonksiyonel, ağrısız, mobil ve düzgün bir ayak elde etmektir. Doğuştan çarpık ayakta talokalka-neonaviküler eklemin açık redüksiyonu oldukça hassas bir cerrahi işlemdir. Postoperatif takip de cerrahi işlem kadar önemlidir. Komplikasyon oranlarını en aza indirmek için operasyonlar deneyimli kişiler tarafından yapılmalı ve postoperatif takibe de özen gösterilmelidir.
