Can Hematological Parameters Be Used For Predicting The Prognosis Of Ischemic Cerebrovascular Disease?

Abstract
Introduction: Cerebrovascular accident (CVA) is a common disease group seen in the emergency department and intensive care unit with high mortality and morbidity levels across the world. In this study, we planned to investigate the markers that can provide an opinion for predicting the prognosis related to this disease. Materials and Methods: The study was conducted retrospectively between 01.03.2014 - 01.09.2015 on patients diagnosed with CVA. Other patients that fulfilled the same criteria in the same period but was not diagnosed with CVA were chosen to be the control group. Patients' hospital stay, APACHE II scores and demographic data were recorded. Mean platelet volume (MPV), platelet/lymphocyte ratio (P/L), neutrophile/lymphocyte ratio (N/L) values from complete blood count draws were recorded on the first, third and the seventh days. C-reactive protein (CRP) and lactate dehydrogenase (LDH) levels were also documented for all patients. Findings: A total of 265 patients were included in the study, of which 92 (34.8%) were in the control group (group 1) and 173 (65.2%) in the CVA group (group 2). During the study period, 350 patients were hospitalized in the intensive care unit and 85 patients were excluded because they did not meet the study criteria. There was no statistically significant difference between the two groups in terms of N/L and P/L ratios. The 3rd and 7th days of MPV were significantly higher in the CVA group (p = 0.001). However, there was no significant effect on mortality and progression prediction. Result: In our study, MPV, N / L and P / L values were not found to be significant in predicting mortality and progression in CVA patients. We believe that more comprehensive prospective studies should be done.

Giriş: Serebrovasküler olay (SVO) acil servis ve yoğun bakım ünitelerinde sık görülen ve dünya çapında mortalite ve morbiditesi yüksek olan bir hastalık grubudur. Bu çalışmamızda bu hastalıkla ilgili prognozu öngörmede fikir verebilecek belirteçleri araştırmayı planladık. Materyal-Metod: Çalışma retrospektif olarak planlanmış olup 01.03.2014 – 01.09.2015 tarihleri arasında olup SVO tanısı almış hastaların dosyaları dahil edildi. Aynı sürede dahil edilme kriterlerini taşıyan ancak SVO tanısı olmayan hastalar kontrol grubu olarak alındı. Hastaların yatış süreleri, APACHE II skorları, demografik verileri kaydedildi. Tam kan sayımında ortalama trombosit hacmi (MPV), platelet / lenfosit oranı (P/L), nötrofil / lenfosit oranı (N/L) değerleri 1. 3. ve 7. günlerde kaydedildi. Ayrıca tüm hastaların C reaktif protein (CRP) ve laktat dehidrogeneaz (LDH) değerleri de kaydedildi Bulgular: Çalışmaya toplam 265 hasta dahil edilmiş olup bunların 92 ‘si (%34,8) kontrol grubu (Grup 1), 173 ‘ü (%65,2) SVO grubu ( grup 2) idi. Çalışma süresi içerisinde yoğun bakıma toplam 350 hasta yatışı olmuş olup 85 hasta çalışma kriterlerini karşılamadığı için dahil edilmedi. Hastaların N/L, P/L oranlarına bakıldığında iki grup arasında istatistiksel olarak anlamlı bir farklılık saptanmadı. MPV’nin 3. ve 7. gün değerleri, SVO grubunda anlamlı olarak yüksek saptandı (p=0,001). Bununla birlikte mortalite ve progresyon öngörme üzerine anlamlı bir etki saptanmadı. Sonuç: Çalışmamızda MPV, N/L ve P/L değerlerinin SVO hastalarında mortalite ve progresyonu öngörmede anlamlı etkisi bulunamamış olmakla birlikte daha geniş kapsamlı prospektif çalışmaların yapılması gerektiği kanaatindeyiz.
