Partial isolated FSH deficiency

Abstract
Objective: In this study, the characteristics of 12 infertile patients with partial isolated FSH deficiency were reviewed. Institution: SSK Ankara Training Hospital, 2n" Urology Outpatient Clinic. Materials and Methods: Isolated partial FSH deficiency was diagnosed in 12 of 1426 patients who had administered our outpatient clinic due to infertility. Semen analysis, serum hormones (FSH, LH, Testosteron, Prolaktin, ostradiol, TSH, Kortizol, and GH), scrotal color Doppler ultrasonography (RDUS), pituitary MRI and karyotype analysis were obtained. The mean age was 29.1 year (25-34) and the mean infertility time was 3.8 year (1.5-10). There was no familial history for hipogonadizm or infertility in any patients. The mean follow-up was 21.4 (11-34) months. Results: Sexual activities, secondary sex characteristics and sense of smell of all the patients were normal. During physical examination, except a patient with clinical varicocele (grade 1), no other genitourinary pathology was detected in the patients. Two patients had oligostenoteratozoospermi (OAT), 9 patients had astenoteratozoospermi, and one patient had isolated teratozoospermia. FSH levels were low in all patients and it was mean of 0.92 (between 0.1-1.3) mIU/ml. The other basal serum hormone levels were within normal male range. In RDUS, testis volumes of the patients were normal and varicocele was detected in two patients (one of whom diagnosed with scrotal examination before). No pituitary pathology was observed in MRI. All patients had normal male karyotypes. Two patients discontinue the follow-up. For the remaining 10 patients, hMG was given to 8 and tamoxifen was given to 2 of the patients. FSH levels reached to a normal after 6 months period in all the patients. Improvement in semen analysis was observed in 6 of the 8 patients who had received hMG treatment, and pregnancies were occurred in three patients' wives. One of the pregnancies was finalized with a normal healthy delivery and one with abortus. The third patient's wife is in 7th month of her pregnancy. Although improvement in semen analysis was detected in both of the 2 patients who received tamoxifen treatment, no pregnancy has been observed in their wives until now. Conclusions: Partial IFSD is a special pathology encountered more often than it was reported in the literature and considering the fact that account of sperm in significant number of patients is normal, it is obvious that spermatogenesis is more complicated that what is thought. FSH treatment appears to be appropriate treatment method.

Amaç: Parsiyel izole FSH eksikliği (İFSD: İsolated FSH Deficiency) saptanan 12 hastaya ait veriler sunulmuştur. Çalışmanın Yapıldığı Yer: SSK Ankara Eğitim Hastanesi 2. Üroloji Polikliniği. Materyal ve Metod: İnfertilite nedeniyle polikliniğimize başvuran 1426 olgunun 2’sinde izole FSH düşüklüğü olduğu tespit edildi. Olgulara semen analizi, hormon (FSH, LH, Testosteron, Prolaktin, Östradiol, TSH, Kortizol, GH), skrotal renkli Doppler ultrasonografi (RDUS), hipofizer MRI ve karyotip analizi tetkikleri yapıldı. Olguların yaş ortalaması 29.1 yıl (25-34), infertilite süresi ortalaması 3.8 yıl (1.5-10) idi. Hiç bir olguda ailesel hipogonadizm ve infertilite hikayesi yoktu. Ortalama takip süresi 21.4 ay (11- 34) idi. Bulgular: Olguların seksüel aktivitesi, sekonder seks karakterleri ve koku alma duyusu normal idi. Fizik muayenede klinik varikoseli mevcut bir olgu (1. derece) dışında başka bir ürogenital patoloji saptanmadı. Semen analizinde 2 hastada oligostenoteratozoospermi (OAT), 9 hastada astenoteratozoospermi, 1 hastada izole teratozoospermi tespit edildi. Olgularda ölçülen 2 FSH seviyesi de normalin altında olup ortalama 0.92 (0.1-1.3 arası) mIU/ml idi. Ölçülen diğer hormon değerleri normal bulundu. RDUS’de olguların testis volümleri normal olup klinik varikoseli mevcut bir hastanın dışında bir olguda daha varikosel bulunduğu tespit edildi. Olguların hipofizer MRI tekiki ve karyotip analizleri normal bulundu. 2 olgu takipten çıktı. Geri kalan 10 olgunun 8’ine hMG, 2’sine tamoxifen tedavileri verildi. Bu olguların tümünde FSH seviyeleri 6. ay sonunda normal değere ulaştı. hMG tedavisi alan 8 olgunun 6’sının semen analizinde iyileşme gözlendi ve 3’ünün eşinde hamilelik gerçekleşti. Bu hamileliklerin biri canlı doğumla, diğeri abortus ile sonlandI. Üçüncü olgunun eşi ise hamileliğinin 7. ayındadır. Tamoxifen tedavisi alan 2 hastanın birinin semen analizinde de iyileşme saptanmış olmakla beraber henüz bu hastaların eşlerinde hamilelik saptanmamıştır. Sonuçlar:Parsiyel IFDS literatürlerde belirtilenlerden daha sık karşılaşılan özel bir patoloji olup hastaların önemli bir kısmında sperm sayısının normal olması spermatogenez sürecinin sanılandan daha komplike bir olay olduğunu göstermektedir. FSH tedavisi uygun tedavi seçeneği olarak gözükmektedir.
