Testicular sperm and intracytoplosmic sperm injection /99 case report

Abstract
The present study is designed to report the experience in sperm extraction from testicular biopsies (TESE) performed between 2001-2004 in men with obstructive and non¬obstructive azospermia in our IVF unit. A total of 99 TESE-ICSI cycles were performed and spermatozoa found in 57 of the patients with a sperm delivery rate 57.5%. ICSI was performed after stimulation with long protocol for 57 patient's wives that spermatozoa was found. The average age of men in this group was 35.4±5.5; the average age of women were 31.1$\pm$5, the FSH, LH and E2 levels of women in unstimulated cycles were 6.4$\pm$2.3, 4.2$\pm$7.9 and 43.9±32.9 respectively. Average number of metaphase II oocytes retrieved per cycles was 10.05±5.6. The fertilization rate was 59%±2.7 and the average embryo number on day 2 was 5.7±4.3 among these patients. Embryos were transferred on day 2 or 3 with an average transferred embryo number of 2.9±1.2. 19 pregnancies were achieved with a pregnancy rate of %33. Among these pregnancies, 16 of them were clinical (28%) and 3 of them were biochemical pregnancy (5%).

Çalışmamızda, merkezimizde 2001-2004 tarihleri arasında obstrüktif ve non-obstrüktif azospermi tanısıyla TESE işlemi uygulanan olgular retrospektif olarak incelendi. 99 olguya yapılan TESE işlemi sonucunda 57 olgudan testiküler sperm elde edilmiş olup sperm elde etme oranı %57 olarak belirlenmiştir. Sperm bulunan 57 olgunun eşlerinin tümüne uzun protokol sonucu ICSI uygulanmıştır. Bu gruptaki erkeklerin yaş ortalaması 35,4±5,5, kadınların yaş ortalaması 31,1 ±5, kadınların FSH, LH ve E2 bazal hormon değerleri sırasıyla 6,4±2,2, 4,1±1,9, 43,9±32,9 olarak belirlenmiştir. Toplam ortalama matür oosit sayısı 10,05+5,6 olarak belirlenmiştir. ICSI işlemi uygulanan yumurtalarda fertilizasyon oranı %59 ±27 ve ikinci gün elde edilen ortalama embriyo sayısı 5,7 ±4,3'tür. Embriyolar ikinci ya da üçüncü gün transfer edilmiş ve ortalama transfer edilen embrio sayısı 2,9±1,2 olarak belirlenmiştir. Toplam 19 gebelik ile %33'lük bir gebelik oranı belirlenmiştir. Bu gebeliklerin 16'sı klinik (%28), 3'U ise biokimyasal olarak meydana gelmiştir.
