A case thought to be consistent with idiopathic thrombocytopenic purpura. Is it misdiagnosis? or resistance to therapy

Abstract
Idiopathic thrombocytopenic purpura (ITP) is a disorder which is associated with thrombocytopenia because of platelet destruction. There may be fault in diagnosis because there is no specific diagnostic test, also there is resistance to medical treatment at a determined ratio. Here, a case of thought has been consistent with ITP was presented who was resistant to classic and high dose steroid at initial treatment, complete remission has been achieved with intravenous immunoglobulin support and splenectomy. The case is still at remission since one and half year. Our aim was to update general knowledge about ITP and emphasize the high resistance rates at therapies and diagnostic difficulties as reported in literature.

İdiyopatik trombositopenik purpura (ITP), platelet yıkımı sonucu trombositopeni ile seyreden bir hastalık olup, spesifik tanısal testi olmadığından tanıda yanılma olabileceği gibi belirli oranda medikal tedaviye direnç olmaktadır. ITP düşünülen bir olguda, başlangıç tedavide klasik doz ve yüksek doz steroide dirençli olup intravenöz immunoglobulin desteği ve splenektomi ile tam remisyon sağlanan ve birbuçuk yıldır halen remisyonda olan bir ITP olgusu sunuldu. Literatür taramasında tedavilerdeki yüksek direnç oranı ve tanıdaki yanılmalar vurgulanarak ITP konusundaki bazı bilgilerin güncellenmesi amaçlandı.
