Abstract

Amaç: Kan serbest ve biyoaktif testosteron seviyelerinin transrektal prostat biyopsisiyle tespit edilmiş yüksek riskli prostat kanserini öngörmedeki değerini saptamak. Gereç ve yöntemler: Yüksek (>2,5 ng/mL) prostat spesifik antijen (PSA) düzeyleri ve/veya anormal dijital rektal muayene bulguları nedeniyle transrektal prostat biyopsisi yapılan 405 hasta çalışmaya dahil edildi. Kan serbest ve biyoaktif testosteron seviyeleri International Society for the Study of the Aging Male (ISSAM) tarafından önerilen formüle göre hesaplandı. Hastalar, PSA seviyeleri ve prostat biyopsisinden elde edilmiş histolojik sonuçlara göre yapılmış D'Amico risk sınıflandırmasına göre benign, düşük risk, orta risk ve yüksek risk gruplarına ayrıldı. Ayrıca hastalar, kanseröz biyopsideki kor yüzdesine göre de 5 gruba ayrıldı. Bulgular: Hastaların 160'ında (%39,5) prostat kanseri saptandı. Toplam, serbest ve biyoaktif testosteron seviyeleri benign ve malign histolojili hastalar arasında anlamlı fark göstermedi. Ancak, ortalama serbest (6,2’e karşı 5,2 ng/dL, p=0,002) ve biyoaktif (151’e karşı 125 ng/dL, p=0,001) testosteron düzeyleri, düşükorta risk prostat kanseri ile yüksek riskli prostat kanseri olan hastalar arasında anlamlı şekilde farklı bulundu. Ayrıca, serbest ve biyoaktif testosteron seviyelerinin kanserli kor oranıyla korele olduğu görüldü (serbest testosteron için p=0,02 ve biyoaktif testosteron için p=0,016). Sonuç: Bu prospektif klinik çalışma, hesaplanmış serbest ve biyoaktif testosteron seviyelerinin düşüklüğü ile yüksek riskli prostat kanseri görülme sıklığı arasında anlamlı bir ilişkili olduğunu göstermektedir. Bu bulgulara göre, kan serbest ve biyoaktif testosteron düzeyleri yüksek riskli prostat kanserini öngörmede yardımcı bir faktör olarak düşünülebilir.

Özet

Objective: To determine the predictive value of free and bioavailable testosterone levels on the detection ofhigh-grade prostate cancer proven by histopathological examination of transrectal prostate biopsy specimens.Material and methods: A total of 405 patients who underwent transrectal prostate biopsy due to high prostaticspecific antigen (PSA) (>2.5 ng/mL) and/or abnormal findings at digital rectal examination were included in thisstudy. Blood free and bioavailable testosterone levels were calculated by the formula recommended by InternationalSociety for the Study of the Aging Male (ISSAM). The patients were stratified according to the D'Amico classificationbased on PSA levels and histological outcomes of prostate biopsies as benign, low, intermediate and high-riskprostate cancer. Patients were also divided into five groups according to the percentage of cancerous cores.Results: Prostate cancer was detected in 160 of 405 (39.5%) patients. Total, free and bioavailable testosteronelevels did not differ significantly between the patients with benign or malign histology. However, meanfree (6.2 vs. 5.2 ng/dL, p=0.02) and bioavailable (151 vs. 125 ng/dL, p=0.001) testosterone levels were foundto be significantly different in men with low-intermediate and high-risk prostate cancer. Moreover, a significantcorrelation was found between free, and bioavailable testosterone levels and percentage of cores withcancer (p=0.002 for free and p=0.016 for bioavailable testosterone, respectively).Conclusion: This prospective clinical study demonstrates that reduced levels of calculated blood free andbioavailable testosterone levels are associated with an increased risk of high-grade prostate cancer. Based onthese findings blood free and bioavailable testosterone levels may be be thought to be an adjunctive factor inthe prediction of high-risk prostate cancer.
