Böbrek nakli sonrası erken dönemde uygulanan prostatın transüretral rezeksiyonu/ insizyonu (TURP/TUIP) sonuçları

Abstract
Amaç: Böbrek nakli sonrası erken donemde mesane çıkım obstrüksiyonuna (MÇO) bağlı gelişen üriner retansiyon nakil başarısını direkt etkileyebilecek sonuçlar doğurabilir. Bu çalışmanın amacı, böbrek nakli sonrası birinci ayda MÇO nedeniyle Transüretral prostat rezeksiyonu (TURP)/Transüretral prostat insizyonu (TUIP) operasyonu yapılan hastaların erken ve uzun dönem takip sonuçlarının değerlendirilmesidir. Gereç ve yöntemler: Şubat 2009 ile Mart 2016 tarihleri arasında 38 erkek hastaya böbrek nakli sonrası ilk 30 gün içinde MÇO’ya bağlı idrar retansiyonu nedeniyle TURP/TUIP operasyonları yapıldı. Tüm hastaların ürodinamik ve böbrek fonksiyon verileri operasyondan önce ve sonraki dönemlerde toplandı. Erken ve uzun dönem komplikasyonlar açısından bütün hastalar minimum 12 ay takip edildi. Sonuçlar retrospektif olarak değerlendirildi. Bulgular: Opere edilen hastaların ortalama yaşı 59,2±12 yıl idi. Ortanca diyaliz süresi 41 (0-180) aydı. Operasyon öncesi ortalama serum kreatin (sCr) düzeyi 1,8±0,7 mg/dL ve Total prostat spesifik antijen düzeyi 1,6±1,1 ng/mL idi. İşeme parameterleri ise maksimum idrar akım hızı (Qmax) ortalama 8,2±4,5 mL/sn, ortalama idrar akım hızı (Qave) 4,6±2,5 mL/sn ve ortanca miksiyon sonrası rezidue idrar miktarı (PMR) ise 105 mL (10-400) idi. TURP/TUIP operasyonları böbrek nakli sonrası ortanca 19 (8-30) günde yapıldı. Operasyonlara bağlı hiçbir hastada major komplikasyon gelişmedi. Postoperatif erken dönemde 5 (%13,1) hastada idrar yolu enfeksiyonu gelişti. TURP/TUIP sonrası birinci ayda ortalama sCr 1,4±0,4 mg/dL’a düşmüştü (p<0,001). İşeme parametreleri değerlendirildiğinde ise ortalama Qmaxve Qave anlamlı şekilde yükselirken (sırasıyla 22,4±11,1 mL/s ve11,7±5,4 mL/s, p<0,001), ortanca PMR ise 15 mL (0-205) ile anlamlı olarak azalmıştı (p<0,001). Hastalar operasyondan sonra ortalama 46,8±23,3 ay takip edildi. Takip süresi içerisinde yalnızca 3 (%7,8) hastada üretra darlığı ve 2 (%5,2) hastada ise mesane boynu darlığı gelişti. Sonuç: Böbrek nakli sonrası ilk ay içinde MÇO 'ya bağlı gelişen üriner retansiyonun cerrahi tedavisinde, TURP/TUIP oldukça başarılıdır, Bununla birlikte kısa ve uzun dönem sonuçları dikkate alındığında düşük morbiditesi nedeniyle güvenle uygulanabilir
Özet
Objective: In the early period after renal transplantation, urinary retention stemming from bladder outletobstruction (BOO) may directly affect graft success. The aim of this study was to evaluate the early andlong-term outcomes of transurethral resection of the prostate (TURP) and transurethral incision of prostate(TUIP) procedures performed in the first month following RT due to BOO.Material and methods: Between February 2009 and March 2016, 38 male patients underwent TURP/TUIP due to BOO within the first 30 days of renal transplantation. The urodynamic and renal function assessmentresults of all patients were collected during the pre-and postoperative periods. All patients werefollowed up for a minimum of 12 months for short and long-term complications. The results were evaluatedretrospectively.Results: The mean age of the patients who underwent operations was 59.2±12 years. The median duration ofdialysis was 41 months (range 0-180). Before the operation the mean serum creatinine (sCr) level was 1.8±0.7mg/dL, the mean total PSA level was 1.6±1.1 ng/mL. Of the voiding parameters, the mean Qmax and Qavewere measured as 8.2±4.5 mL/sec and 4.6±2.5 mL/sec, respectively. The median post-micturition residualurine (PMR) was 105 mL (range 10-400). TURP/TUIP operations were performed at a median of 19 days(range 8-30) after renal transplantations. None of the patients experienced major complications. In the earlypostoperative period, 5 patients (13.1%) developed urinary tract infection. The mean decrease in sCr in thefirst month following TURP/TUIP was 1.4±0.4 mg/dL (p<0.001). The mean Qmax (22.4±11.1 mL/sec), andQave (11.7±5.4 mL/sec) increased significantly (p<0.001), while the median PMR (15 mL, range 0-205)decreased significantly (p<0.001). The mean follow-up period after the procedure was 46.8±23.3 months.During the follow-up period, 3 (7.8%) patients suffered from urethral stricture and 2 (5.2%) patients frombladder neck obstruction.Conclusion: In the surgical treatment of urinary retention arising from BOO in the first month followingrenal transplantation, TURP/TUIP yield safe and successful results. In addition, regarding the short andlong term outcomes, these procedures may be safely performed with low morbidity
