Abstract
Retrograd intrarenal cerrahi (RİRC) üriner sistem taş hastalığının tedavisinde etkili ve güvenli bir seçenek olmuştur. Fleksibl üreterorenoskoplar ve yardımcı ekipmandaki son gelişmeler ve yenilikler bu prosedürün artan başarı oranları ile daha kolay ve etkin yapılabilmesine olanak sağlamıştır. RİRC 2 cm’den küçük taşı olan, daha önce ekstrakorporeal şok dalga tedavisinin başarısız olduğu, infundibüler stenozu, renoüreteral malformasyonu, iskelet-kas sistemi deformitesi, kanama diyatezi olan ve obez hastalarda primer tedavi yöntemi olarak kullanılabilmektedir. RİRC ile ilgili bu detaylı derlemenin ikinci bölümünde taş kompozisyonunun başarı oranına etkisi, preoperatif taşsızlık oranının değerlendirilmesi, tedavi maliyeti, RİRC eğitimi, operasyon sırasında floroskopi kullanımı, RİRC’in çeşitli taş tiplerinde ve özel hasta gruplarındaki rolü ve de kombine tedavi yöntemlerinde RİRC’in yeri güncel literatür bilgisi ile tartışılmıştır
Özet
Retrograde intrarenal surgery (RIRS) has become an effective and safe treatment modality in the managementof urinary system stone disease. Recent developments and innovations in the flexible ureterorenoscopeand auxiliary equipment have made this procedure easier and more effective with increased success rates.RIRS can be used as a primary treatment in patients with renal stones smaller than 2 cm, prior unsuccessfulshock wave lithotripsy, infundibular stenosis, renoureteral malformation, skeletal-muscular deformity,bleeding diathesis and obese patients. In the second part of this detailed review for RIRS, effect of stonecomposition on success rate, preoperative assessment of stone-free rate, the cost of this modality, educationfor RIRS, fluoroscopy use, the current role of RIRS in the treatment of various urolithiasis types and specialconditions, and combined treatment methods are discussed with up-to-date literature.
