Abstract

Varikosel ilerleyici bir hastalıktır ve varikosele bağlı testiküler hipotrofi insidensi puberte ile birlikte artmaktadır. Bu derlemede adolesan varikoselin güncel tedavisi ve paterniteyle birlikte fertilite potansiyeli irdelenmektedir. Adolesan varikoselde güncel tedavi endikasyonu etkilenen testiste %20 hacim kaybı ve pubertal dönemde bozulmuş sperm parametreleri varlığıdır. Varikoselde en iyi tedavi yöntemi varikoseli tamamen düzeltmesi yanında çok düşük komplikasyon oranlarına sahip olmalıdır. Mikrocerrahi varikoselektomi konvansiyonel yöntemlere göre en az komplikasyon oranına sahiptir. Bu derlemede ayrıca varikosel tedavisi uygulanan, paternite oranlarını bildiren ve kontrol grubuna sahip 2 çalışmanın meta-analiz sonuçları verilmektedir. Mikrocerrahi varikoselektomi uygulanan çocuklar kontrol grubuna göre 2,42 kat artmış paternite oranına sahiptir. (p değeri: 0,000). Özet

Varicocele is a progressive disease, and the incidence of varicocele and varicocele related testicular hypotrophyincreases with puberty. This review updates novel management and future paternity potential inadolescents with varicocele. Main goal of treatment of the adolescent varicoceles is preservation of fertility.Current recommendations for adolescent varicocele repair are testicular hypotrophy (>2 mL or >20%) onthe affected side and/or impaired spermatogenesis in the post-pubertal period. The best treatment modalityfor adolescent varicocele should include elimination of the varicocele with lower complication rates. Microsurgicalvaricocelectomy techniques have been shown to result in fewer recurrences and postoperativecomplications than conventional varicocelectomy in adolescents. We conducted a meta-analysis, including2 studies with the control groups, and reporting paternity situation after varicocele treatment. In this metaanalysis,varicocele repair increases paternity rate 2.42 times with a p value of 0.000. Adolescents withvaricocele and their families should be counselled in the light of these findings.
