Pediyatrik vezikoüreteral reflüde kontrastlı işeme ürosonografisiyle, işeme sistoüretrografisinin karşılaştırılması

Abstract
Amaç: İşeme sistoürografisinin (VCUG) vezikoüreteral reflü (VUR) tanısında altın standart teknik olduğu düşünülür. Ancak çocukları radyasyona maruz bırakan floroskopi kılavuzluğuna gerek vardır. İşeme ürosonografisi (VUS) teknik olarak VCUG’a benzemekle birlikte radyasyon maruziyeti yoktur. Bu çalışma VUR tanısında VCUG’a göre kontrastlı VUS’un (ce-VUS) etkinliğini belirlemeyi amaçlamaktadır. Gereç ve yöntemler: Bu çalışmaya 3 yıllık dönemde 30 çocuk dahil edilmiştir. Hastaların tümüne aynı gün VCUG, ardından ce-VUS yapılmıştır. VUS tetkiklerinin tümü sonografi bölümünde aynı sonologist tarafından uygulanmıştır. Raporlamadan önce tüm görüntüler aynı sonografist tarafından kaydedilmiş ve gözden geçirilmiştir. Bulgular: Hastaların ortanca yaşı 51,53 ay idi. Yirmi bir erkek ve 9 kız hasta vardı. VCUG ile 16 hastada reflü yoktu, 14 hasta ise reflülüydü. ce-VUS’da 14 hastada reflü yoktu, 16 hastada ise reflü vardı. Toplam 58 böbreküreter ünitesinin (BÜÜ) 17’sinde VCUG ve 21’inde ce-VUS reflü saptamıştı. ce-VUS, VCUG’de görülemeyen 4 reflü olgusunu saptamıştı. Ce-VUS 3 sağ BÜÜ’de VUR saptarken, VCUG belirleyememişti. Yirmi sekiz sol BÜÜ’nin 25’inde ce-VUS, VCUG ile VUR’un derecesi açısından uyum göstermişti. Üç VUR da ise VCUG ile VUR arasında uyum yoktu. Ce-VUS iki BÜÜ’de VCUG’a göre bir derece yüksek ve bir BÜÜ’de de bir derece düşük VUR saptamıştır. Toplamda ceVUS, VCUG’un atladığı 4 olguyu saptamıştır. Sonuç: VCUG karşılaştırıldığında pediyatrik VUR'u değerlendirmede ce-VUS, üstün tanısal performansı, uygulanabilirliği ve çocukların radyasyondan güvenle korunması açısından iyi bir görüntüleme tekniğidir
Özet
Objective: Voiding cystourethrography (VCUG) has been considered as the gold standard technique for thediagnosis of vesicoureteral reflux (VUR). But, it requires fluoroscopic guidance which expose children toradiation. Voiding urosonography (VUS) is technically analogous to VCUG and has the major advantage ofzero radiation exposure. This study aims to determine the efficacy of contrast enhanced-VUS (ce-VUS) withrespect to VCUG in diagnosing VUR.Material and methods: This study involves 30 children over a period of 3 years. All patients underwent aVCUG followed by the ce-VUS on the same day. All VUS studies were done by the same sonologist in thesonography department. The images were recorded and reviewed by the same sonologist before reporting.Results: The median age of the patients was 51.53 months. There were 21 males and 9 females. On VCUG,16 patients had no reflux, and 14 patients had reflux. On ce-VUS, 14 patients had no VUR, and 16 patientshad VUR. Of the total 58 kidney-ureter units (KUUs), VUR was detected in 17 KUUs on VCUG and in 21KUUs on ce-VUS. Thus, ce-VUS detected 4 refluxing units that were not seen on VCUG. In right KUUs,ce-VUS detected VUR in 3 units where no reflux was found in VCUG. In the 28 left KUUs, 25 units on ce-VUS showed concordance with the grade of VUR as detected by VCUG; 3 were discordant. Two units once-VUS showed a VUR one grade higher than the corresponding grade on VCUG and in one unit it was onegrade lower. Thus, in total, ce-VUS picked up 4 cases which were missed by VCUG.Conclusion: ce-VUS is a good imaging modality when compared to voiding cystourethrography to assesspediatric vesicoureteral reflux, in view of its superior diagnostic performance, feasibility and radiationsafety for children.
