The Effect of Early Complications on Flap Selection on Sacral Pressure Sores

Abstract

Objective: Pressure sores occur in bedridden patients in intensive care units, clinics, and even at their own places. Care for sick relatives and working with doctors to address treatment options and ensure proper follow-up are some of the problems associated with these types of wounds. Surgical therapy in the treatment of pressure ulcers is associated with significant complications. In comparison to non-surgical treatment, surgical treatment has a low complication rate and is more cost-effective. The surgical treatment enables the patients to return to their social life sooner than non-surgical treatment. Patient's ability to early return to their social life is advantageous in terms of reducing morbidity and the need for additional operations. This study is aimed to review the flap choices used to treat sacral pressure sores and the resulting acute complications rates retrospectively.Material and Methods: Patients treated for stage 3 and stage 4 sacral pressure sores in our clinic in the past 5 years were included in the study. Patient records were analyzed retrospectively. Patients' demographic data and surgical treatment they received were documented. Surgical method and surgical outcomes were evaluated and early complication rates were determined.Result: Fifty patients were included in the study; 10 of them were female, 40 of were male cases. The most common causative agent was paraplegia after traffic accident.Conclusion: A significant difference was not observed between the type of flap used in the surgical treatment and the rate of complications. Consequently, the surgical treatment of pressure ulcers in the sacral region depends on the patient's individual situation, the cooperation of the family, and previously applied treatments.

Amaç: Bası yaraları tüm yataklı servislerde, yoğun bakımlarda ve yatağa bağımlı olan hastalarda ortaya çıkan, hasta, hasta yakınları ve hekim için bakım, tedavi ve takibi problem oluşturan yaralardandır. Cerrahi tedavi bası yaralarının tedavisinde önemli bir yere sahiptir. Uzun dönem cerrahi dışı tedaviye kıyasla düşük komplikasyon oranlarına sahip cerrahi tedaviler maliyet etkinliği, hastanın sosyal hayatına erken dönmesi, ek operasyon ihtiyacını ve morbiditeyi azaltması açısından avantajlıdır. Bu çalışmada sakral bölgedeki bası yaraları için uygulanan flepler, komplikasyon oranları açısından geriye dönük olarak karşılaştırılmıştır.Gereç ve Yöntemler: Kliniğimizde son beş yılda tedavileri yapılan evre 3 ve evre 4 sakral bası yarası hastaları çalışmaya dâhil edildi. Hasta kayıtları geriye dönük olarak incelendi. Hastaların demografik verileri ve uygulanan cerrahi tedaviler kayıt altına alındı. Operasyon yöntemi ve oluşan erken komplikasyonlar değerlendirildi.Bulgular: Çalışmaya dâhil edilen olguların 40 tanesi erkek iken 10 tanesi kadındı. En sık görülen etiyolojik neden trafik kazası sonrası parapleji idi.Sonuç: Komplikasyon oranları açısından yapılan flepler arasında anlamlı bir fark görülmedi. Sonuç olarak sakral bölgedeki bası yaralarının cerrahi tedavisinde hastanın bireysel durumu, ailenin kooperasyonu ve daha önce uygulanan tedaviler göz önünde bulundurulmalıdır.
