Effect of hypercalciuria on bone mineral density in patients with nephrolithiasis

Abstract

Objective: The aim of this study was to investigate the effect of hypercalciuria on the loss of bone density and to assess the occurrence of osteopenia and osteoporosis in patients less than fifty years of age with nephrolithiasis. Material and Methods: A total of 50 patients (22 premenopausal female, mean age 34.7 ± 7.1 years, and 28 male, mean age 36.0 ± 7.7 years) with nephrolithiasis were included. Bone mineral density (BMD) measurements were performed by Dual-energy X-ray Absorptiometry method in posteroanterior position at the L1-L4 vertebrae and femoral neck regions. Results were obtained as BMD (g/cm2), T- and Z-score. Because of gender effect on BMD, the values are expressed according to sex. Results: Mean L1-4 vertebrae bone mineral density (BMD) was 1.138 ± 0.130 g/cm2 and femoral neck BMD was 1.005 ± 0.132 g/cm2 in women with nephrolithiasis and 1.083 ± 0.129 and 1.017 ± 0.120 g/cm2 for men respectively. According to T-score, osteopenia was detected in 3 women (6%) and 11 men (22%). Osteoporosis was detected in 1 man (2%). According to z-score, low bone mineral density was found 5 (10%) patients. Hypercalciuria was detected in 13 (26%) patients. However, no statistically significant difference was detected in terms of BMD, T and Z-scores between patients with and without hypercalciuria. The decrease of BMD (osteopenia) was detected in 6 hypercalciuric patients and 9 normocalciuric patients had osteopenia and 1 patient had osteoporosis according to T-scores. Conclusion: We found that patients with nephrolithiasis may have low bone mineral density with or without hypercalciuria, so nephrolithiasis must be interrogated in examination in young adults as one of the causes of bone mineral loss.

Amaç: Bu çalışmanın amacı idrarla kalsiyum atılımındaki artışın kemik kaybına etkisini araştırmak ve 50 yaş altı böbrek taşına sahip hastalarda osteopeni ve osteoporoz sıklığını tespit etmekti. Gereç ve Yöntemler: Çalışmaya böbrek taşı olan 22’si premenapozal kadın (34.7 ± 7.1 yıl), 28’i erkek (36.0 ± 7.7 yıl) toplam 50 hasta dahil edildi. Kemik mineral yoğunluğu (KMY) ölçümleri Çift Enerjili X-ışını Absorpsiyometri yöntemi ile posteroanterior pozisyonda L1-L4. vertebralardan ve femur boyun bölgesinden ölçüldü. Sonuçlar KMY (g/cm2), T-skor and Z-skor olarak elde edildi. Cinsiyetin KMY üzerindeki etkisi yüzünden değerler cinsiyete göre verildi. Bulgular: Böbrek taşına sahip kadınlarda lomber 1-4 vertebra KMY ortalaması 1.138 ± 0.130 g/cm2, femur boynu kemik mineral yoğunluğu 1.005 ± 0.132 g/cm2 ve bu değerler erkeklerde sırasıyla 1.083 ± 0.129 ve 1.017 ± 0.120 g/cm2 olarak saptandı. T-skoruna göre osteopeni 3 (%6) kadın, 11 (%22) erkekte ve osteoporoz 1 (%2) erkek hastada saptandı. Z-skoruna gore düşük kemik mineral dansitesi 5 hastada (%10) bulundu. Hiperkalsiüri 13 (%26) hastada saptandı. Hiperkalsiürisi olan hastalar ile olmayanlar arasında KMY, T-skor ve Z-skorları arasında istatistiksel olarak anlamlı fark tespit edilmedi. T-skoruna göre KMY’nda azalma (osteopeni) 6 hiperkalsiürik hastada tespit edildi ve dokuz normoklasürik hasta osteopeniye ve 1 hastada osteoporoza sahip idi. Sonuç: Biz hiperkalsiüri olsun veya olmasın böbrek taşı hastalarının düşük kemik mineral yoğunluğuna sahip olabileceğini bulduk. Bu sebeple böbrek taşı, genç erişkinlerde kemik mineral kaybının bir sebebi olarak incelenmelidir.
