A Study on Awareness and Knowledge Level About Risky Situations in Pregnancy: Can Pregnant Schools Be an Effective Way?

Abstract
Introduction: A significant part of maternal deaths ocur because of late recognizing the problem and apply to health institutions. The aim of this study is to evaluate the effects of trainings on “danger signs during pregnancy” given to pregnant women in different institutions on awareness of risky situations in pregnancy. Method:The study was conducted with a total of 312 participants, 163 from pregnant policlinics and 149 from pregnant schools. The study data were collected using the data form designed by the researchers. The data form includes questions that relatedwith participant's soyodemographic and obstetric characteristics, and their knowledge level about danger signs in pregnancy, and questions designed in consideration of the Ministry of Health Antenatal Care Method Guidelines to evaluate the content of prenatal care services. Results:The mean age of the participants was found to be 28.5 ± 5.3 years. 52.2% (n = 163) of the participants were included in the study from the pregnancy out patient clinic and 47.8% (n = 149) from the pregnant school. While 65.1% of those who participated in the study from the pregnancy school were followed by both the family doctor and the obstetrician, this rate was 50.3% (p = 0.005) in the pregnant out patient clinic. 85.2% of the participants from the pregnancy school has sufficent prenatal knowledge but this rate was 4.9% for pregnancy polyclinics (p = 0.000). The participants were asked whether they would consult the emergency service or their physicians if they experience one of the danger signs, and a statistically significant difference was found between the two groups, which developed suddenly developing edema (p = 0.000) and high fever (p = 0.003). There was no statistically significant difference in other parameters. Conclusion:It was found that those who were included in pregnant schools had a significantly higher level of knowledge, higher prenatal follow-up rates, and use iron, folic acid and vitamin D supplements more regularly. Pregnant schools increase compliance with the recommendations given on pregnant women.

Giriş: Anne ölümlerinin önemli bir kısmı gebelerin ilgili sorunu geç tanıması ve sağlık kuruluşlarına geç başvurması nedeniyle gerçekleşmektedir. Buçalışmanın amacı gebelere değişik kurumlarda verilen “gebelikteki tehlike işaretleri” ile ilgili eğitimlerin gebelikteki riskli durumların farkındalığına etkisinideğerlendirmektir. Yöntem: Çalışma gebe polikliniğinden 163 ve gebe okullarından 149 gebe olmak üzere toplam 312 katılımcı ile gerçekleştirilmiştir.Çalışma verileri araştırmacılar tarafından oluşturulan veri formu ile toplanmıştır. Veri formu katılımcının sosyodemografik, obstetrik özellikleri, gebeliktetehlike işaretleri ile ilgili bilgi düzeylerini ölçen ve doğum öncesi bakım hizmetlerinin içeriğini değerlendirmeye yönelik olarak Sağlık Bakanlığı DoğumÖncesi Bakım Yöntemi Rehberi dikkate alınarak düzenlenmiş soruları içermektedir.Bulgular: Katılımcıların yaş ortalaması 28,5±5,3 yıl olarakbulunmuştur. Katılımcıların %52,2’si (n=163) gebe polikliniğinden, %47,8’i (n=149) ise gebe okulundan çalışmaya dahil edilmişlerdir. Gebe okulundançalışmaya katılanların %65,1’i hem aile hekimi hem de kadın doğum hekimi tarafından takip edilirken bu oran gebe polikliniğinden çalışmaya alınanlarda%50,3’tür (p=0,005). Gebe okulundan katılanların %85,2’sinin prenatal bilgi düzeyi yeterli bulunmuşken, gebe polikliniklerinden katılanlarda bu oran%4,9’da kalmıştır (p=0,001). Katılımcılara tehlike işaretlerinden birini yaşamaları durumunda acile ya da hekimlerine başvurup vurmayacakları sorulmuşher iki grup arasında ani gelişen ödem (p=0,001) ve yüksek ateş (p=0,003) istatistiksel olarak anlamlı fark bulunmuştur. Diğer parametrelerde istatistikselaçıdan anlamlı fark gösterilmemiştir. Sonuç: Gebe okullarından dahil edilenlerde belirgin olarak daha yüksek bilgi düzeyine sahip olunduğu, daha yüksekprenatal takip oranlarına ulaşıldığı, demir, folik asit ve D vitamini desteklerini daha düzenli kullandıkları bulunmuştur. Gebe okulları gebeler üzerindeverilen önerilere uyumu arttırmaktadır.
