Whole Lung Lavage in a Pulmonary Alveolar Proteinosis Patient with Severe Respiratory Failure

Abstract
Pulmonary alveolar proteinosis (PAP) is a rare interstitial lung disease that develops as a result of defects in the clearance of sur- factant by alveolar macrophages. The accumulation of lipid- and protein-rich substances in the alveoli constitutes the main pathol- ogy of this disease. PAP has three types of aetiology: autoimmune (primary), congenital and secondary. The most common form in adults is autoimmune PAP. Whole lung lavage is a commonly per- formed method for treatment of this form of disease, especially in more severe cases. Performed under general anaesthesia, the mate- rial deposited in the alveoli is removed by washing. In this paper, we present a whole lung lavage under anaesthesia in a PAP patient who had severe respiratory failure.
Özet
Pulmoner alveoler proteinozis (PAP), surfaktanın alveol makro- fajlarınca temizlenmesindeki eksiklik sonucunda oluşan nadir gö- rülen bir interstisyel akciğer hastalığıdır. Bu hastalığın ana patolo- jisini lipid ve proteinden zengin maddenin alveol içinde birikimi oluşturur. PAP etyolojik olarak otomimmün (primer), kalıtsal ve sekonder olmak üzere üç tipe ayrılmaktadır. Erişkinlerde en sık görülen şekli otoimmün PAP'dır. Bu şekilde, özellikle şiddetli olgularda sıklıkla uygulanan tedavi tüm akciğer lavajıdır. Genel anestezi altında uygulanan bu işlemde, alveollerde biriken ma- teryalin yıkama ile uzaklaştırılması amaçlanmaktadır. Bu yazıda şiddetli solunum yetmezliği olan PAP hastasında anestezi altında uygulanan tüm akciğer lavajı sunulmuştur.
