Primary extramedullary plasmacytoma of the liver

Abstract

Primary extramedullary plasmacytoma of the liver is a very rare tumor. We report the fifth case of primary extramedullary plasmacytoma of the liver. A 22 year-old woman who presented with abdominal pain and hepatomegaly, had a hepatic mass, 12 cm diameter, involving both lobes of the liver in abdominal ultrasonography and computed tomography. Pathologic diagnosis of liver biopsy was plasmacytoma. IgG kappa monoclonal gammopathy was detected. Bone marrow examinations and bone survey were normal. The size and location of the tumor was not suitable for complete resection or deliverance of effective doses of radiotherapy. VAD (Vincristine: 0,4 mg/day by continuous infusion (CI) x 4 days; adriamycin: 9 mg/m2/day by CI x 4 days; dexamethasone: 40 mg po on days 1-4, 9-12, 17-20) combination chemotherapy was started. Paraproteinemia disappeared at the end of the sixth cycle. After eight cycles of treatment, the liver mass was seven cm in diameter and located in one lobe of the liver. At this stage surgery was performed. Tumor was not found in any of the pathology specimens. The patient is disease free and is in very good condition postoperatively at 39th month.
