A case with carcinoid tumor located on carina and with complete atrioventricular block treated by right tracheal sleeve pneumonectomi

Abstract

In a 45-year-old male with complete A-V block, the mass lesion located on the carina and causing serious tracheal obstruction was removed with right tracheal sleeve pneumonectomy and was histopathologically diagnosed as typical carcinoid tumor. In the resection material of right upper lobectomy performed 15 years ago due to bronchiectasy, carcinoid tumor was identified in the lobe bronchus. Because of recurrence of the carcinoid tumor 15 years later, co-presence of A-V complete block due to carcinoid cardiopathy, and the application of surgical treatment with bronchoplastic method as tracheal sleeve pneumonectomy, the case has been presented with the related literature.

Özet

Kırk beş yaşında, atriyoventriküler (A-V) tam blok gelişmiş erkek olguda karinaya yerleşerek ciddi trakeal obstrüksiyon yapan kitle lezyonu sağ trakea “sleeve” pnömonektomiyle çıkarılmış ve histopatolojik olarak tipik karsinoid tümör tanısı konulmuştur. On beş yıl önce bronşektazi nedeniyle yapılan sağ üst lobektomi materyalinde lob bronşunda karsinoid tümör tespit edilmiştir. On beş yıl sonra karsinoid tümör rekürrensi olması, A-V tam blok gibi nadir olabilecek karsinoid kardiyopati özelliğinin birlikteliği, trakea “sleeve” pnömenektomi gibi bronkoplastik yöntemle cerrahi tedavi uygulanması nedeniyle, olgu ilgili literatürler eşliğinde sunulmuştur.
