Results of tuberculin skin tests performed in the students of primary school in Eskişehir and microfilms of tuberculin skin test positive students and their family members

Abstract

In this study, BCG screening and tuberculin skin test (TT) were performed in 2417 first class primary students from three regions of Eskişehir city in which habitants' socioeconomical and sociocultural levels differed; the suburbs of the city (group 1), the city center (group 2) and the private school population (group 3). At least one BCG scar was found in 2172 (89.9%) students but weren't present in 245 (10.1%). TT was found positive in 41 (1.7%), negative in 2376 (98.3%) students. TT endurations were increasing with BCG scare number in students (p<0.001). TT endurations were correlated with BGG scare number in students and family members (r=0.333, p=0.000 and r=0.221, p=0.024 respectively). Infection prevalence was found as 0.91% in group 1, 0.25% in group 2 and 0% in group 3. Natural infection prevalence was 5.9%, 4.4% and 0% respectively. The ratio of annual risk of infection was 0.86%, 0.64% and 0% respectively. The microfilms of 41 students with TT positive were taken and their family members were examined. TT was positive in 76 (73%) and negative in 28 (27%) family members. Pulmonary lesion such as hiler enlargement and fibrotic scar ratios were found higher in students and family members with $\geq$20 mm TT enduration than $\leq$20 mm TT enduration.

Özet

Bu çalışmada Eskişehir'deki sosyoekonomik ve kültürel yönden farklı olan gecekondu bölgesinde (1. bölge), şehir merkezinde (2. bölge) ve özel okullarda (3. bölge) 2417 ilkokul birinci sınıf öğrencisine BCG taraması ve tüberkülin deri testi (TT) yapılıp, sonuçları değerlendirildi. Öğrencilerin 2172'sinde (%89.9) en az bir BCG skarı vardı, 245'inde (%10.1) ise BCG skarı yoktu. TT 41 (%1.7) öğrencide pozitif, 2376 (%98.3) öğrencide ise negatif idi. BCG skar sayısı ile orantılı olarak TT endürasyon çapı artıyordu ve TT endürasyon çapının öğrencilerde ve aile bireylerinde BCG skar sayısı ile bağıntılı olduğu saptandı (sırasıyla; r=0.333, p=0.000 ve r=0.221, p=0.024). İnfeksiyon prevelansı 1. bölgede %0.91, 2. bölgede %0.25 ve 3. bölgede %0 olarak saptandı, bu oranlar doğal infeksiyon prevalansında %5.9, %4.4 ve %0, yıllık infeksiyon riskinde ise %0.86, %0.64 ve %0 olarak saptandı. TT pozitif olan 41 (%1.7) öğrencinin mikrofilmleri çekildi ve bunların aile bireyleri muayene edildi. TT aile bireylerinin 76'sında (%73) pozitif ve 28'inde (%27) negatif olarak saptandı. Hiler genişleme ve fibrotik skarlar gibi akciğer patolojileri, TT endürasyon çapı 20 mm ve üzerinde olan öğrencilerle aile bireylerinde, TT endürasyon çapı 20 mm ve altında olanlara göre yüksek oranlarda saptandı.
