Diagnostic value of CA 125 in the diagnosis of tuberculosis pleurisy

Abstract
The aim of our study was to compare the diagnostic value of CA 125 as an additive to ADA in the diagnosis of tuberculosis pleurisy cases compared with pleural biopsy. Sixty six pleural effusion cases, 51 male and 15 female, were included in our study. Forty two tuberculous pleurisy cases diagnosed due to clinical, radiological and histopathological evidence constituted the tuberculosis group, while 24 cases with effusion due to a nontuberculous eti- ology constituted the control group. In the tuberculosis group, which has a definitive diagnosis with pleural biopsy, pleural effusion, CA 125 and ADA, when evaluated together, did not have an effect on the diagnosis (p>0.05). Likewise, the pleural effusion, ADA and serum CA 125 values had no effect on the diagnosis when compared with the pleural biopsy (p>0.05). However, the pleural effusion ADA value alone has a high sensitivity and specificity (90.5% and 95.8% respectively). Its positive predictive value was calculated as 97.4% and negative predictive value was calculated as 85.2%. When the calculated cut-off CA 125 value alone in the pleural effusion was 992.8, it had a sensitivity of 92.9% and specificity of 25 %. In these circumstances, the positive predictive value was 68.4%, and negative predictive value was 66.7%. In conclusion, in the diagnosis of tuberculous pleurisy in countries where tuberculosis prevalence is high, as in our country, pleural effusion ADA value is a valuable diagnostic test, but neither CA 125 alone, nor serum or pleural effusion CA 125 in combination with ADA, have an additive diag- nostic value.

Bu çalışmada tüberküloz plörezi tanısında yüksek sensitivite ve spesifiteye sahip olduğu bilinen ADA aktivitesinin yanına eklenen CA 125 ölçümünün, biyopsi ile kıyaslanarak tanı değerlerinin ortaya konması amaçlandı. Çalışmamıza, 51'i erkek, 15'i kadın olmak üzere toplam 66 plevral effüzyonlu olgu alındı. Klinik, radyolojik ve histopatolojik özellikleri ile tüberküloz plörezi tanısı alan 42 olgu tüberküloz grubunu oluştururken, tüberküloz dışı etiyolojilere bağlı 24 olgu ise kontrol grubu olarak kabul edildi. Plevral biyopsi ile kesin tüberküloz tanısı elde edilen tüberküloz grubunda plevral sıvı CA 125 ve ADA düzeyleri birlikte bakıldığında tanıya katkısı olmadığı görüldü (p>0.05). Benzer şekilde plevral sıvı ADA ve serum CA 125 ölçümleri de biyopsi ile kıyaslandığında tanıya katkı sağlamadılar (p>0.05). Ancak tek başına plevral sıvı ADA ölçümü yüksek sensitivite ve spesifite gösterdi (sırasıyla %90.5 ve %95.8). Bu ölçümün pozitif prediktif değeri %97.4 ve negatif prediktif değeri %85.2 olarak hesaplandı. Tek başına plevral sıvıda CA 125'in belirlenen 992.8 cut-off değeri için sensitivitesi %92.9 ve spesifitesi de %25 olarak bulundu. Bu durumda pozitif prediktif değer %68.4, negatif prediktif değer ise %66.7 olarak hesaplandı. Tek başına serumda bakılan CA 125'in belirlenen 52.5 cut-off değeri için sensitivitesi %70.7, spesifitesi ise %45.8 olarak bulundu. Bu değerler için pozitif prediktif değer %69 ve negatif prediktif değer ise %47.8 olarak hesaplandı. Sonuç olarak tüberküloz plörezi tanısında ülkemiz gibi yüksek tüberküloz prevalansına sahip ülkelerde plevral sıvı ADA aktivitesinin değerli bir tarama testi olduğu, tek başına ya da ADA tayinine eklenen serum veya plevral sıvı CA 125 ölçümünün tanıya kakısı olmadığı ortaya konuldu.
