An evaluation on the improvement of infantmortality rate and total fertility rate in our country

Abstract
International Institutions like OECD, World Bank and WHO publish peridically the indicators of the counties on social, economic and health matters. Turkey is taking place at the end of ordered lists of many indicators while she is recording the highests improvents in three of those indicators, namely Total Fertility Rate, İnfant Mortality Rate and Life Expectancy at Birth. While all other indicators are bad, it is suspicious to have good improvements on theese indicators. The values of Indicators like TFR and IMR, are high when the health system is absentor it exists but ineffective. By the establishment of an effective health system, these indicators begin to fall rapidly at the beginning, as time goes on the rate of falling gets slower and at the end both of these indicators reach at their natural minimum level. It is almost impossible to have lower levels than natural levels. In 1966-67 the value of TFR was 5.30 children while IMR was 131 per thousand. With the effective application of the "Law on the Socialization of Health", (Lawnumber 224), TFR fell to 2.61 and IMR to 43 per thousand. The great success on these problems was attained between 1970-90 in Turkey. Beyond this point, It is expected to have slower improvements on theese indicators. But, unexpectedly, TFR fell to 2.23 in the year 2003 and to 2.16 in 2008. IMR was 53 per thosand in 1993 and 43 in 1998, unexpectedly it fell to 23 per thosand in 2003 and to 17 in 2008 and 10.1 in 2010. Extraordinary imrovements in TFR and IMR makes us think that the data used to calculate these indicators are unreliable. In this paper, it is aimed to discuss there liability of indicators basing on objective proofs.

OECD, Dünya Bankası ve Dünya Sağlık Örgütü gibi örgütler periyodik olarak ülkelerin sosyal, ekonomik ve sağlık göstergelerini yayımlamaktadırlar. Türkiye Uluslararası Göstergelerin pek çoğunda ülkeler sıralamasında sonlarda yer alırken üç tanesinde rekor iyileşmeler sağlamış gibi görülmektedir. Bunlar toplam doğurganlık hızı (TDH),bebek ölüm hızı (BÖH), Doğuşta yaşam beklentisi göstergeleridir. Diğer göstergeler kötü iken bu göstergelerin iyi olması ister istemez rekor iyileşmeler konusunda kuşku yaratmaktadır. TDH ve BÖH gibi göstergeler; halk sağlığı düzenlemelerinin yokluğunda ya da etkisizliğinde yüksek düzeyde seyrederken, etkin bir hizmet sisteminin kurulmasıyla başlangıçta kısa zamanda hızla düşmeye başlar. Zamanla bu düşüş yavaşlar, sonunda her iki gösterge de doğal düzeyine erişir, doğal düzeyinden daha aşağı inmesi de olası değildir. 1966-67 yıllarında TDHortalama 5.30 çocuk, BÖH da binde 131 idi. Türkiye'de 224 sayılı Sağlığın Sosyalleştirilmesi Yasası'nın kurduğu etkin sağlık sistemi sayesinde 1998'de TDH 2.61'e, BÖH binde 43'lere düşmüştür. Bu göstergelerdeki "başlangıçtaki büyük değer düşüşü" Türkiye'de 1970-1990 arasında sağlanmıştır. 1990'dan sonra, bu göstergelerdeki düşüşün yavaşlayan bir hızda devam etmesi beklenir. Ancak, TDH, 2003 yılında beklenmedik bir şekilde 2.23'e düşerken; bunu izleyen 5 yılda da durağan nüfus düzeyi olan 2.1'e düşmüştür. BÖH'nın değeri de 1993'te 53'e, 1998'de de 43'e düşmüştür. Daha sonra beklenmeyen bir hızla 2003'de binde 23'e; 2008'de binde 17'ye, 2010'da binde 10.1'e düşmüştür. TDH ve BÖH değerlerinde görülen beklenmedik düşüşler, bu göstergelerin üretiminde baz olan verilerin güvenilir olmadığını düşündürmektedir. Makalede, bu güvensizliğin nesnel kanıtları verilmekte ve tartışılmaktadır.
