The influence of neoliberal policies on tobacco control and health education

Abstract

Educating society is an important component of health promotion. However, in recent years, as a result of the increased influence of neoliberal hegemony, health is perceived as an individual rather than a social issue. Thus health promotion strategies are focused at the individual level from a biomedical perspective. The field of tobacco control also has been influenced by neoliberal policies. In this light, the world tobacco epidemic is seen as the result of "bad individual choices," and health education and health promotion policies use "victim blaming" approaches. However, the tobacco epidemic is a consequence of an insatiable hunger for profit by transnational tobacco companies. The neoliberal policy of privatizing tobacco monopolies in developing countries has enabled transnational tobacco companies to control additional tobacco markets, leading to an increased in tobacco use. The emphasis on the individual overlooks the importance of neoliberal polices and the social and physical context of the tobacco epidemic. We have tended to look for solutions inside the very system that is the source of the problems. To be successful, tobacco control programs should not just aim to change individual behavior. Our efforts should also address the physical, social, and cultural environment. This article refutes biomedical reductionism in tobacco control and focuses on problems at the system level from the perspective of health education and promotion. Özet

Toplumun bilgilendirilmesi sağlığın geliştirilmesi sürecinin önemli parçalarından birisidir. Ancak neoliberal hegemonyanın etkisiyle sağlık, son yıllarda giderek artan biçimde bireylerin sorunu olarak algılanmakta ve çözüm önerileri de bu algı çerçevesinde biyomedikal perspektife indirgenerek "birey"e sunulmaktadır. Ne yazık ki tütün kontrolü alanı da neoliberal ideolojiden fazlasıyla etkilenmiştir. Bu bakış açısı nedeniyle dünyada yaşanan "tütün salgını"nın nedeni bireylerin yanlış tercihleriymiş gibi gösterilmekte ve sağlık eğitimi ve sağlığı geliştirme politikaları "kurbanı suçlayan" bir yaklaşımla belirlenmektedir. Oysa dünyada yaşanan tütün salgınının nedeni çokuluslu sigara şirketlerinin doymak bilmez kar hırsıdır. Öte yandan devlet TEKEL'lerinin özelleştirilerek tütün piyasasının tümüyle çokuluslu şirketlerinin kontrolüne geçmesi, var olan salgını arttıran başka bir neoliberal politikadır. Fakat tüm bu gerçeklere rağmen, tütün salgının küresel sonuçları, neoliberal politikalara ve bireylerin ve toplumların yaşadıkları fiziksel ve sosyal çevreye vurgu yapılmadan gündeme gelmekte ve sistemin var ettiği soruna sistem içerisinde "çözüm" üretilmeye çalışılmaktadır. Oysa tütün kontrolünün başarıya ulaşması için bireyin davranışını hedefleyen eğitim programlarının yanı sıra, sistem boyutunda fiziksel, sosyal ve kültürel çevreyi değiştirmeyi hedefleyen politikalar da hayata geçirilmelidir. Bu makale, tütün kontrolü alanında, biyomedikal indirgemeciliği reddederek, sağlık eğitimi ve sağlığı geliştirme perspektifinde soruna sistem boyutunda odaklanmaktadır.
