Endosaccular Onxy injection and endovascular treatment with parent artery reconstruction technique in cerebral aneurysms

Abstract
PURPOSE: The aim of this study was to test the feasibility and efficacy of the treatment of wide-necked and giant intracranial aneurysms with a new liquid embolic agent. MATERIALS AND METHODS: During injection of Onyx, an occlusion balloon with or without an intracranial stent was inflated in the parent artery. Sixty-seven patients harboring 72 intracranial aneurysm, most of which being giant or wide-necked, were succesfully treated with this technique. RESULTS: Seventy of 72 aneurysms were internal carotid artery aneurysms. Of these, 37 were ophthalmic (53%), 18 were cavernous (26%) and 15 were petrous (21%) internal carotid artery aneurysms. One patient had 2 aneurysms located at the basilar and superior serebellar arteries. Fifty-three aneurysms were treated with the balloon remodelling technique and 19 were treated with the use of an intracranial stent. Recanalisation was found in 8 of 15 giant aneurysms which were treated with the balloon remodelling technique and 3 of these underwent a second treatment in which stents were used. Mortality occurred in 3 patients. CONCLUSION: Onyx embolisation with balloon or stent remodelling technique is a feasible and effective method in the treatment of high-risk giant or wide-necked intracranial aneurysms which are difficult to treat with other endovascular or microsurgical techniques.

AMAÇ Mikrostent veya balon koruma sağlanarak Onyx ile intrakranyal anevrizmaların oklüzyonunun teknik olarak uygulanabilirliğinin ve bu tekniğin özellikle geniş boyunlu ve dev anevrizmaların oklüzyonundaki başarısının saptanması. GEREÇ VE YÖNTEM Semptomatik ya da asemptomatik serebral anevrizma nedeni ile endovasküler tedavi için değişik merkezlerden refere edilen, çoğu anevrizmanın geniş boyunlu ve dev boyutta olması nedeniyle cerrahi tedavisi çok zor ya da mümkün olmayan 67 hasta çalışmaya dahil edildi. İşleme başlamadan önce anevrizmanın boynuna balon koruma veya koroner stent uygulanıp uygulanmayacağı kararlaştırıldı. Onyx ile oklüde edilen 70 internal karotid anevrizmasının 37'si oftalmik (%53), 18'i kavernöz (%26) ve 15'i de petröz (%21) segmentteydi. Bir hastada baziler ve süperior serebellar arterde 2 anevrizma mevcuttu. Anevrizmaların 53'ünde Onyx uygulaması balon koruma, 19'unda ise stent kullanılarak yapıldı. Balon koruma ile tedavisi yapılan 15 büyük-dev anevrizmalı hastanın 8'inde kontrol anjiyogramlarda anevrizma boynundan tekrar büyüme saptandı ve bu hastalardan 3'üne stent kullanılarak Onyx'le 2. seans tedavi uygulanarak tekrar büyüme saptanan anevrizmaları tedavi edildi. Bu 3 hastada 2. seans tedavi sonunda tam oklüzyon sağlandı. Hiçbir hastada işleme bağlı tromboembolik olay olmadı. Toplam 3 hastada mortalite gelişmiş olup bunların 2 'si işleme bağlıydı (%3). SONUÇ Anevrizma boynuna balon koruma ve stent uygulanarak yapılan Onyx embolizasyonu başta diğer endovasküler ya da mikrocerrahi tekniklerle oldukça riskli hatta imkansız olan geniş boyunlu ve dev anevrizmalar olmak üzere, intrakranyal anevrizmaların tedavisinde uygulanabilir ve etkili bir yöntemdir.
