Importance of lipid peroxides in pathogenesis of preeclampsia

Abstract

Şişli Etfal Hospital, Biochemistry and Clinical Biochmistry Preeclampsia is an endothelial disease of which pathogenesis and etiology is still an open question. Today, preeclampsia is the major reason of fetal immaturity, premature births and maternal deaths. Reasons of the endothelial damage in preeclampsia are low blood protacycline levels, low plathelet counts, increased lipid peroxidation. This leads to exceed the capacity of the antioxidant systems of the body. The aim of the study was investigate the reletionship between plazma lipid peroxide level and pathogenesis of preeclampsia. Our study groupconsisted of 40 preeclamptic preganades and 20 healty pregnant women, whom we have primarily measured plasma lipid peroxide level and secondarily measured serum cholestrol, triglyceride, uric acid, total protein, albumin, blood plathelet count, hematocrit, andfibrinogen levels. We used Thiobarbituric acid method. In this methon malodialdehi.de, end product of the lipid peroxidatian, reacts with thioharbutiric acid and produces pink reaction end product. We have observed that plasma lipid peroxide level showed a blood pressure dependent increase, where both diastolic and systolic blood pressures were highly correlated with, lipid peroxside levels (p<0.001, rl= 0.79, r2= 0.83). We have also observed that lipid peroxide levels in preeclamptic pregnancies were significantly higher than that of the healty pregnant subjects (p<0.0001). When we clasified the peeclamptic patients according to the severity of the desease, as mild and severe, the higher increased was found in the severe preeclamptic group. Considering the information that in preeclamptic patients lipid peroxides indirectly caused endothelial damage by affecting the prostaglandin metabolism, distrupt the platelet membrane and lead to vasocontriction. Özet

Preeklampsi fizyopatolojisi ve etyolojisi hala tümüyle açıklanmamış bir endotel bozukluğudur. Günümüzde fetal gelişme geriliği, prematüre doğum ve maternal ölümlerin başta gelen nedenlerinden biridir. Preeklampside görülen; endotel hasan, düşük prostasiklin kan düzeyleri, trombositlerin agregasyonu, düşük trombosit sayısı ve vazospazm, hipertansiyon, proteinüri ve ödem gibi fizyolojik ve metabolik değişikliklerde artmış lipid peroksidasyonunun ve kandaki antioksidanlarm koruyucu kapasitelerinin aşılmasının, rol oynadığı gösterilmiştir. Biz bu çalışmada, plâzma lipit peroksit düzeyi ile preeklampsi patogenezi arasındaki ilişkiyi araştırdık. 40 preeklamptik gebe ile 20 sağlıklı gebenin plazma lipit peroksit düzeyi ile serum kolesterol, trigliserit, ürik asit, total protein, albumin, tam kan trombosit sayısı, hematokrit vefibrinojen düzeylerini ölçtük. Plazma lipit peroksit tayininde, lipid peroksidasyonunun son ürünü olan MDA (malodialdehite)'in, TBA (thiobarhitürik asit) ile oluşturduğu pembe rengin ölçümüne dayanan TBA yöntemi kullanılmıştır. Plazma lipit peroksit düzeyi ile diastolik ve sistolik kan basınçları arasında pozitif korelasyon bulunduğunu saptadık (p<0.001, rl=0.79, 1-2=0.83). Preeklamptik olgularda lipit peroksit düzeyinin kontrol grubuna göre anlamlı derecede arttığını gözledik (p<0.0001). Hastalarımızı sistolik ve diastolik kan basınçlarını dikkate alarak gruplandırdığımızda, en yüksek artışın ağır grupta olduğunu gördük. Preeklampsi olgularında lipit peroksitlerin dolaylı olarak prostaglandin metabolizmasını bozarak endotel hasarı ve trombosit membran hasan ile vazokonstrüksiyona yol açabileceği bilgisi ışığında, lipit peroksitlerin hastalığın primer semptomu olan hipertansiyona katkıda bulunabileceği sonucuna vardık.
