The Comparison of Synchronized Intermittent Mandatory Ventilation with Nonsynchronised Intermittent Mandatory Ventilation in Newborn with Respiratory Failure and Transient Tachypnea

Abstract

Aim: We aimed to compare the efficacy of synchronized intermittent mandatory ventilation (SIMV) with non-synchronised intermittent mandatory ventilation (NIMV) in the treatment of respiratory failure of newborns who have transient tachypnea (TTN) and their short term results.Materials and Methods: This study was carried out prospectively. For the treatment of respiratory failure in patients who had been diagnosed with TTN and had respiratory failure, SIMV and NIMV support was randomly applied. In patient groups receiving SIMV and NIMV, respiratory rate, oxygen saturation, blood gas parameters and ventilator support duration, tachypnea duration and short term complications were recorded at the outset and at the first hour of ventilator treatment.Results: No significant difference was determined in terms of demographical features, mechanical ventilator support duration, tachypnea duration, mechanical ventilator complications and mortality between the groups. Also there was no considerable difference between the groups with regard to oxygen saturations and blood gas parameters checked prior to attachment to the mechanical ventilation and at the first hour of mechanical ventilation support. Respiratory rate was (73.72±6.74/min and 69.65±4.93/min; p=0.022) at the first hour of mechanical ventilation support and average respiratory rate checked during the support was (67.44±4.27/minute vs 64.73±3.13/min; p=0.025) and it was established as significantly higher in the NIMV group than the SIMV group. Conclusion: We determined that NIMV support was as efficient as SIMV support in the treatment of respiratory failure in patients with TTN together with respiratory failure and it did not lead to an increase in the complications. New studies are required to use NIMV effectively and confidently in the treatment of respiratory failure in babies with TTN.

Amaç: Çalışmamızda yenidoğan geçici takipneli (YDGT) bebeklerde solunum yetmezliği tedavisinde senkronize aralıklı zorunlu ventilasyonun (synchronize intermittent mandatory ventilation-SIMV) ile senkronize olmayan nazal aralıklı zorunlu ventilasyonun (nonsynchronize intermittent mandatory ventilationNIMV) etkinliği ve kısa dönem sonuçlarının karşılaştırılması amaçlandı.Gereç ve Yöntemler: Bu çalışma prospektif olarak yapıldı. YDGT tanılı ve solunum yetmezliği olan hastalarda solunum yetmezliği tedavisi için randomize olarak SIMV ve NIMV desteği uygulandı. SIMV ve NIMV alan hasta gruplarında, ventilatör tedavisi başlangıcında ve ventilatör tedavisinin birinci saatinde solunum sayısı, oksijen satürasyonu, kan gazı parametreleri ile birlikte ventilatör destek süresi, takipne süresi ve kısa dönem komplikasyonları kaydedildi.Bulgular: Gruplar arasında demografik özellikler, mekanik ventilatör desteği süresi, takipne süresi, mekanik ventilatör komplikasyonları ve mortalite açısından anlamlı fark saptanmadı. Mekanik ventilatöre bağlanma öncesinde ve mekanik ventilatör desteğinin birinci saatinde bakılan oksijen satürasyonları, kan gazı parametrelerinde de gruplar arası anlamlı fark saptanmadı. Mekanik ventilatör desteğinin birinci saatinde ki solunum sayısı (73,72±6,74/dk ve 69,65±4,93/dk; p=0,022) ve mekanik ventilatör desteği boyunca bakılan ortalama solunum sayısı (67,44±4,27/dk ve 64,73±3,13/dk; p=0,025) NIMV grubunda SIMV grubuna göre anlamlı yüksek saptandı.Sonuç: Solunum yetmezliği olan YDGT hastalarında solunum yetmezliği tedavisinde NIMV desteğinin SIMV desteği kadar etkili olduğunu ve komplikasyon artışına sebep olmadığını saptadık. NIMV'nin YDGT'li bebeklerde solunum yetmezliği tedavisinde etkili ve güvenle kullanılabilmesi için yeni çalışmalara ihtiyaç vardır.
