Diagnosis of vertebrobasilar insufficiency due to unilateral vertebral artery hypoplasia with color Doppler sonography

Abstract

Purpose: Vertebral artery hypoplasia (VAH) may be defined by measurement of vertebral artery flow volume using color duplex sonography. Flow volume below than 30 to 40 mL/min is generaly accepted as cut of points for definition of VAH. In this study we aimed to find whether hypoplasia of unilateral ver-tebral artery always causes vertebrobasilar insufficiency or not. Methods: We examined 29 patients with VAH (12 women, 17 men; mean age, 51±16 years). None of patients had carotid artery stenosis. We detected flow volume of vertebral arteries using color Doppler sonography, and net vertebral artery volume was determined as the sum of flow volumes of both vertebral arteries. Vertebral artery flow volume below than 40 mL/min was accepted as vertebral artery hypoplasia. The association of VAH and vertebrobasilar insufficiency was investigated. Results: Net vertebral artery flow volumes were severely damped (below 120 mL/min) in 20 of 29 patients (69%), moderately damped (120-200 mL/min) in 7 of 29 patients (24%), and normal (above 200 mL/min) in 2 of 29 patients (7 %). Conclusion: Patients who have net vertebral artery flow volume below 200 mL/min are prone to become symptomatic with vertebrobasilar iscemia. In our study, we found damped vertebral artery flow volume in 27 of 29 patients and the net vertebral artery flow volume was compensated by the other vertebral artery and reached to 200 mL/min level only in 2 patients (7%). However more studies with larger series are needed to determine the compensation ratios in patients with unilateral VAH.

Özet

Amaç: Vertebral arter hipoplazisi (VAH), renkli dupleks sonografi ile vertebral arter volümleri ölçülerek saptanabilir. Genel olarak vertebral arter volümünün 30-40 mL/dk' dan düşük olması VAH olarak kabul edilmektedir. Bu çalışmada tek taraflı VAH saptanmış olguların vertebrobaziler yetmezliğe neden olup olmadığını araştırdık. Gereç ve Yöntem: Çalışmamıza 12 kadın ve 17 erkek olmak üzere toplam 29 hasta (ortalama yaş 51±16) dahil edildi. Tüm hastalarda dupleks sonografi kullanılarak bilateral vertebral arter volümleri saptandı. Vertebral arter volümü 40 mL/dk altında olan ve vertebral arter çapı uniform tarzda 2 mm'nin altında olan olgular VAH olarak kabul edildi. Bu olgularda vertebrobaziler yetmezlik ve VAH birlikteliği araştırıldı. Bulgular: Yirmidokuz hastanın 20'sinde ileri dereceli (120 mL/dk altında), 7 hastada orta dereceli (120-200 mL/dk) vertebrobaziler yetmezlik saptanırken 2 hastada ise normal vertebral arter volümü (200 mL/dk ve üzeri) bulundu. Sonuç: Toplam vertebral arter voümü 200mL/dk'nın altında olan olgularda vertebrobaziler yetmezlik semptomları olabileceği bilinmektedir. Bizim çalışmamızda VAH saptanan olguların 27'sinde vertebrobaziler yetmezlik ile uyumlu akım volümü saptanırken 2 olguda kompensasyon olduğu saptandı (%7). Ancak bu oranın geniş serilerle yapılan çalışmalarla teyid edilmesi gerektiğini düşünüyoruz.
