Thrombotic Brachial Artery Occlusion After Stent Implantation in a Case With Aortic Coarctation

Abstract

Coarctation of the aorta is a congenital condition that decreases the life expectation, may cause serious clinical results, and high mortality and morbidity rates, if it is not treated. Currently, surgical repair, balloon dilatation and stent implantation with percutaneous way are used in the treatment of the coarctation, effectively. Although the percutaneous interventional treatment is the least invasive procedure, both percutaneous intervention and surgery have some complications. Multidetector computed tomography (MDCT) angiography is a safe and reliable imaging modality that can be used to assess the area of coarctation after the stent placement procedure and follow up the complications. In this report, we aimed to present MDCT angiography findings of a case having occlusion at the left brachial artery after placement of the stent to the coarctation area. We thought local vasoconstriction and/or endothelial damage at the site of the left brachial artery access area as the reason of the occlusion. MDCT angiography provided a detailed information about both stent site and the brachial artery occlusion area at the same time. Thus it provided a significant contribution to the diagnosis and treatment protocols

Özet

Aort koarktasyonu, yaşam beklentisini düşüren, ciddi klinik sonuçlar doğurabilen, tedavi edilmediğinde yüksek mortalite ve morbiditeye neden olan konjenital bir hastalıktır. Günümüzde bu patolojinin tedavisinde cerrahi düzeltme, perkütan yaklaşım ile balon dilatasyonu ve stent yerleştirme uygulamaları etkin olarak kullanılmaktadır. Perkütan girişim ile stent yerleştirilmesi koarktasyonun tedavisinde en az invaziv yöntem olsa da, hem cerrahi düzeltme işlemlerinin hem de perkütan tedavinin çeşitli komplikasyonları raporlanmıştır. Çok kesitli bilgisayarlı tomografi (ÇKBT) anjiyografi bu hastalarda işlem sonrası koarktasyon alanının kontrolü ve olası komplikasyonların değerlendirilmesinde güvenle kullanılabilecek etkin bir radyolojik görüntüleme yöntemidir. Bu yazıda, femoral ve sol brakiyal arter yoluyla girişim yapılarak başarılı şekilde perkütan stent tedavisi uygulanan aort koarktasyonlu bir olguda endotel hasarı ve/veya lokal vazokonstrüksiyona ikincil oluştuğu değerlendirilen trombotik sol brakiyal arter oklüzyonunun ÇKBT anjiyografi bulgularını sunmayı amaçladık. ÇKBT anjiyografi aynı anda hem stent alanının hem de brakiyal arter oklüzyonunun değerlendirilmesine olanak sağlayarak tanıya ve tedavi planlamasına önemli katkıda bulunmuştur
