The comparison of chronic delta hepatitis patients with HB antigen negative chronic hepatitis B patients

Abstract

Background/Aims:The hepatitis delta virus (HDV) is a small defective virus. Delta hepatitis is the least common form of chronic viral hepatitis but is the form most likely to lead to cirrhosis. We aimed to research the role of hepatitis delta virus infection in chronic liver diseases related to hepatitis B virus infection and to compare with HBe antigen negative Hepatitis B patients in the Van region of Eastern Turkey. Materials and Methods: 17(16 male an one female) patients with chronic hepatitis D and 30 patients (21 male and 9 female) with chronic hepatitis B were prospectively analysed and compared.by clinicoepidomiologic findings. Liver biopsy was available in all the patients. Scoring of liver histology was performed according to the classification of Knodell by a blinded pathologist. Results: Hepatitis delta virus infection was detected in 36% (17/47) of HBe antigen negative chronic hepatitis B patients. Chronic hepatitis D virus infection was more frequent in males (p<0.001). Mean ages of chronic hepatitis D and HBe antigen negative chronic hepatitis B patients were 30.7±8 (14- 65), and 32.4±13 (16-52), respectively. Liver biopsy results were similar in two groups.Knodell HAI was 10.7(4-16) in Hepatitis B group and was 11.5 (5-17) in Delta hepatitis group. The positivity were significantly higher in Van compared to Western Anatolia. Conclusion: The higher prevalence of hepatitis delta virus in HBe antigen negative chronic hepatitis B patients suggests that hepatitis delta virus infection increases the severity of disease and should be detect early. The present study indicates that delta hepatitis is Delta virus could be identified by a liver biopsy and/or HDV- RNA assay at initial presentation especially in younger asiatic male patients .

Özet

Amaç: Hepatit D virüsü küçük bir defektif virüstür. Delta hepatiti kronik viral hepatitlerin enaz görülen formu olmasına rağmen ençok siroza yol açan etkendir. Bizler; çalışmamızda kronik karaciğer hastalarında hepatit delta virüs enfeksiyonunun Van Bölgesindeki rolünü ve HBe antijen negatif Kronik Hepatit B hastalarından farkını araştırmayı amaçladık. Materyal ve metodlar: Kronik Delta Hepatitli 17 hasta ( 16 erkek ve bir kadın ) ve HBe antijen negatif Kronik Hepatit B’li 30 hasta ( 21 erkek ve 9 kadın)prospektif olarak incelendi. Tüm hastalara karaciğer biyopsisi yapıldı. Karaciğer histolojisi Knodell sınıflaması temelinde tek bir patolog tarafından değerlendirildi. Bulgular: Hepatit Delta virüs enfeksiyonu 47 HBe Antijen Negatif Kronik Hepatit B hastasının %36’sında (17 hasta) saptandı. Kronik Delta hepatiti erkeklerde baskındı. ( p 0.005) Kronik Hepatit B hastalarında Delta virüs enfeksiyonu oranı Batı Anadolu bölgesine göre oldukça yüksektir. Sonuç: Hepatit delta virüs enfeksiyonu HBe Antijen Negatif Kronik Hepatit B hastalarında mortaliteyi artırdığından dolayı erkenden tanınması gereklidir. Özellikle genç, Asyalı ve erkek Hepatit B hastalarında PCR ve karaciğer biyopsisi yolgöstericidir.
