Abstract

Gastric carcinoma following surgery for benign gastroduodenal disease is being increasingly reported. A high incidence of carcinoma in gastric remnants following partial gastrectomy and other gastric operations has been reported. The lack of sphincteric function at the gastroenterostomy stoma with consequent reflux of bile containing intestinal contents into the stomach has been cited as a cause of mucosal abnormalities and malignant changes in the stomach. Also, Helicobacter pylori (Hp) gastritis following these operations aid in malignant change by stimulating cell proliferation. Consequently, it would be useful to predict the incidence of Hp infection in operated stomach and to give eradication therapy. The recommended treatment for gastric cancer is total gastrectomy, D2 lymph node dissection and omentectomy.

Özet

Bening mide ve duodenum hastalıkları nedeni ile yapılan cerrahi müdahaleyi takiben ortaya çıkan mide kanserleri gittikçe artan sayıda rapor edilmektedir. Mide ameliyatlarından sonra epitelyal hasara neden olan safra reflüsii ve Helieobacter pylori gastritine bağlı artmış hücre proliferasyonu sonucu mide kanserlerinin görülme olasılığı artmaktadır. Dolayısı ile öpere midelerde H. Pylori sıklığını bilmek ve gerekirse eradikasyon tedavisi uygulamak akılcı bir yaklaşım olabilir. Mide güdük kanseri cerrahisinde günümüzde kabul edilen cerrahi tedavi yöntemi total gastrektomi D2 lenf nodu diseksiyonu ve omentektomidir.
